从“熄风定悸”探讨心律失常的辨治

摘　　要：心律失常是心血管疾病的常见病,临床治疗中抗心律失常药物及射频消融、植入性心律转复除颤器等治疗手段,均因其副作用或适应证的限定等因素,限制了其在临床上的普及,目前中医药治疗心律失常仍有不可替代的优势。笔者依据多年的理论探讨及临床治疗经验,结合心悸病证的发病病机、病症特点等方面,提出心悸病证属内风,熄风定悸是临床有效的治疗方法。探讨以熄风定悸方为主要基础,结合阴阳气血病机所变,对兼有气虚、血瘀、肝郁、阴阳亏虚不同病机的心悸证,提出了益气熄风、熄风化瘀、疏肝宁心、养阴熄风、温阳定悸的治疗大法,为中医药辨治心悸提供了新思路。Arrhythmia is a common cardiovascular disease.In clinical treatment,anti-arrhythmic drugs,radiofrequency ablation,and implantable cardioverter defibrillators are all limited in their application due to side effects or restricted indications.Currently,traditional Chinese medicine still has irreplaceable advantages in the treatment of arrhythmia.Based on years of theoretical exploration and clinical treatment experience,and considering the pathogenesis and characteristics of palpitations,the author proposes that palpitations belong to internal wind,and suppressing wind to stabilize palpitation is an effective clinical treatment method.Based on the Xifeng Dingji Formula as the main foundation and combined with the pathogenesis of yin-yang,qi and blood,this paper proposes the treatment principles of benefiting qi and suppressing wind,suppressing wind and resolving stasis,soothing the liver and calming the heart,nourishing yin and suppressing wind,and warming yang and stabilizing palpitations for the syndrome of palpitations with different pathogenesis such as qi deficiency,blood stasis,liver depression,and deficiency of yin-yang,providing a new idea for the differentiation and treatment of palpitations with traditional Chinese medicine.
