基于“虚气留滞”理论治疗偏头痛验案举隅

摘　　要：偏头痛是一种常见的神经系统疾病,通常表现为单侧、搏动性头痛,伴随恶心、呕吐、对光和声音敏感等症状。其发作频率和严重程度因人而异,可能持续数小时到数天。偏头痛常见于女性,尤其是在青春期和更年期阶段。然而其病程长,极易反复发作,用药多,西药临床治疗效果常不尽如人意。中药具有多靶点、多途径的特点,临床效果明确且副作用小的优势在治疗偏头痛中占有重要地位。刘金民教授根据其30余年的临床经验发现,目前偏头痛发病率较高,且有明显的家族遗传倾向,融合传统中医与现代临床理念,以“虚气留滞”理念对偏头痛形成一套独有的诊疗体系,认为偏头痛核心病机为元气内虚,邪气留滞,本虚标实,治疗应经络循行引经用药,充分运用虫类药,并强调针药并用,循经祛邪。Migraine is a common neurological disorder,typically characterized by unilateral,pulsatile headaches accompanied by symptoms such as nausea,vomiting,sensitivity to light and sound.The frequency and severity of its attacks vary from person to person,and may last for hours to days.Migraine is common in women,especially during adolescence and menopause.However,its disease course is long,prone to recurrent attacks,requires a lot of medication,clinical treatment effect of western medicine is often unsatisfactory.Chinese materia medica has the characteristics of multiple targets and pathways,its advantages of clear clinical effects,and minimal side effects play an important role in the treatment of migraine.Based on more than 30 years of clinical experience,Professor Liu Jinmin finds that the current incidence rate of migraine is high,and there is an obvious familial genetic tendency,integrating traditional Chinese medicine with modern clinical concepts,a unique diagnosis and treatment system for migraine is formed based on the theory of“stagnation of deficiency of qi”.It is believed that the core pathogenesis of migraine is internal deficiency of vital energy,stagnation of evil qi,deficiency in origin and excess in superficislity,treatment should use drugs along channels and meridians,make full use of insect drugs,and emphasize combination of acupuncture and medicine to eliminate evil along channels.
