磁共振成像检查诊间预约爽约影响因素及应对策略分析

摘　　要：目的分析磁共振成像检查患者爽约的原因,探讨降低爽约率的应对策略,以提高临床影像检查资源的利用效率。方法从首都医科大学附属北京同仁医院医技预约分诊管理服务平台、医院信息系统收集2024年6月至7月进行磁共振成像检查数据及爽约数据,分析患者爽约的影响因素。采取等距抽样方式对爽约患者进行电话回访,了解其爽约原因。结果共有12825人次进行磁共振成像检查,发生爽约1127人次。不同来源、年龄、检查预约时间及检查是否跨院区、小程序线上改约前后患者爽约率比较,差异有统计学意义(P<0.05)。logistic回归分析显示,来源急诊(OR=149.552,95%CI:48.053~465.434,P<0.001)、来源住院(OR=212.799,95%CI:66.828~677.617,P<0.001)、检查是否跨院区(OR=0.846,95%CI:0.732~0.978,P=0.024)、小程序线上改约(OR=1.176,95%CI:1.036~1.334,P=0.012)和检查预约时间(OR=1.005,95%CI:1.002~1.007,P<0.001)是磁共振成像检查患者爽约的影响因素。结论跨院区预约和预约时间较长患者更易发生爽约,可通过线上改约、检查前提醒及相应约束机制提高磁共振成像检查效率。Objective To analyze the reasons of magnetic resonance imaging examination patients appointment failure,explore coping strategies for reducing appointment failure rate,and improve utilization efficiency of clinical imaging examination resources.Methods The data of magnetic resonance imaging examination and appointment failure from June to July 2024 at medical technology appointment triage management service platform and hospital information system in Beijing Tongren Hospital,Capital Medical University were collected.Influencing factors of patients appointment failure were analyzed.The equidistant sampling method was adopted to conduct telephone follow-up visits on patients with appointment failure,in order to understand the reasons of appointment failure.Results A total of 12825 cases underwent magnetic resonance imaging examination,with approximately 1127 cases of appointment failure.There were statistically significant differences in appointment failure rates of patients with different sources,ages,examination appointment times,whether examinations were cross hospital,before and after online rescheduling of mini program(P<0.05).Logistic regression analysis showed that emergency department(OR=149.552,95%CI:48.053-465.434,P<0.001),hospital admission(OR=212.799,95%CI:66.828-677.617,P<0.001),cross hospital examination(OR=0.846,95%CI:0.732-0.978,P=0.024),mini program online rescheduling(OR=1.176,95%CI:1.036-1.334,P=0.012),and appointment time(OR=1.005,95%CI:1.002-1.007,P<0.001)were the influencing factors of appointment failure in magnetic resonance imaging examination patients.Conclusion Cross hospital appointments and longer appointment times are more likely to result in missed appointments for patients.Online rescheduling,pre examination reminders,and corresponding constraint mechanisms can be used to improve the efficiency of magnetic resonance imaging examinations.
