经尿道前列腺钬激光剜除术与经尿道前列腺电切术治疗良性前列腺增生患者的临床效果及对前列腺功能、性功能的影响比较

摘　　要：目的观察经尿道前列腺钬激光剜除术(holmium laser enucleation of the prostate,HoLEP)与经尿道前列腺电切术(trans urethral resection prostate,TURP)治疗良性前列腺增生(benign prostatic hyperplasia,BPH)患者的临床效果及对前列腺功能、性功能的影响。方法对照组(n=34)给予TURP治疗,观察组(n=34)给予HoLEP治疗,对比两组手术相关指标,前列腺症状(IPSS、QOL)及性功能(IIEF-5、EHGS)评分,并发症(继发性出血、尿失禁、尿路感染及膀胱刺激征)的发生风险。结果与对照组比较,观察组各项手术指标均更优(t=-11.903,P=0.000;t=-11.801,P=0.000;t=-16.369,P=0.000)。治疗后,所有患者的IPSS评分与QOL评分均下降(P<0.05),且观察组优于对照组(t=-14.349,P=0.000;t=-7.969,P=0.000);IIEF-5评分及EHGS评分均升高(P<0.05),观察组优于对照组(t=9.098,P=0.000;t=8.563,P=0.000)。两组术后并发症发生率较观察组更低(χ^(2)=5.314,P=0.021)。结论HoLEP治疗BPH临床效果良好,可有效减少术中出血量,缩短恢复时间,改善患者前列腺症状及性功能状态,安全性高,值得推广。Objective To observe the clinical effects of holmium laser enucleation of the prostate(HoLEP)and transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia(BPH)patients and their impacts on prostate function and sexual function.Methods The control group(n=34)was treated with TURP,while the observation group(n=34)was treated with HoLEP.The surgical-related indicators,prostate symptoms(IPSS,QOL),and sexual function(IIEF-5,EHGS)scores,as well as the occurrence risks of complications(secondary bleeding,urinary incontinence,urinary tract infection,and bladder irritation symptoms)were compared between the two groups.Results Compared with the control group,the observation group had better surgical indicators(t=-11.903,P=0.000;t=-11.801,P=0.000;t=-16.369,P=0.000).After treatment,the IPSS and QOL scores of all patients decreased(P<0.05),and the observation group was superior to the control group(t=-14.349,P=0.000;t=-7.969,P=0.000);the IIEF-5 and EHGS scores increased(P<0.05),and the observation group was superior to the control group(t=9.098,P=0.000;t=8.563,P=0.000).The postoperative complication rates of the two groups were 5.88%and 26.47%,respectively,and the observation group was lower(χ^(2)=5.314,P=0.021).Conclusion HoLEP has a good clinical effect in the treatment of BPH,can effectively reduce intraoperative blood loss,shorten recovery time,improve prostate symptoms and sexual function of patients,and has high safety,which is worthy of promotion.
