摘　　要：意识障碍患者的评估是临床护理与康复治疗中的关键环节。现有的意识障碍评估工具存在诸多局限,如操作复杂、主观性强、缺乏针对性,导致临床应用中难以快速、准确地判断患者的意识状态,进而影响病情监测和治疗决策的及时性与有效性。本研究设计制作一种意识障碍程度判断简易尺,具有操作简便、直观易用、成本低廉的特点,能够快速、准确地评估患者的意识状态,同时便于护理人员统一描述与记录,设计和应用充分贴合临床需求,具有较高的实用价值。The assessment of patients with consciousness disturbance is a crucial part in clinical nursing and rehabilitation treatment.The existing assessment tools for consciousness disorders have many limitations,such as complex in operation,subjective outcome measures,and lack of Specificity.As a result,it is difficult to quickly and accurately determine the consciousness status of patients in clinical practice,thereby affecting the timeliness and effectiveness of disease monitoring and treatment decisions.This paper introduced a simplified ruler for assessing the degree of disturbance of consciousness.With simple operation,intuitive usability and low cost features,it can quickly and accurately assess the patient's state of consciousness.At the same time,it is convenient for nursing staff to describe and record uniformly.
