乳糜胸的外科治疗

摘　　要：目的　总结乳糜胸外科治疗经验。方法　回顾近 1 4年收治乳糜胸 1 8例。其中平均乳糜量超过 65 0 0ml者 1 6例 ,低于 2 5 0 0ml者 2例。采用剖胸低位胸导管结扎或缝扎手术 1 6例 ,单纯行胸腔闭式引流术 2例。结果　剖胸手术的 1 6例中有 1 5例及胸腔闭式引流的 2例一次性治愈 ,1例剖胸手术病人术后复发 ,经二次剖胸后方治愈 ,全组病人随访 3个月 - 1年 ,乳摩胸未见复发。结论　乳摩胸若每日量超过 1 0 0 0ml者 。Objective To review the experience on surgical treatment of chylothorax.Methods From December 1987 to December 2001,18 patients with chylothorax underwent thoracic surgery.The average amount of chyle in 16 cases is more than 6 500 ml,that of the others is less than 2500 ml.2 cases were treated by thoracic closed drainage,with the others by ligation or string suture of thoracic duct with inferior positioning Results 15 cases in 16 cases of ligation of thoracic duct and 2 cases of drainage were cured.1 case of ligation was recurred postoperation,and was cured by the second ligation.There was no recurrence during follow up 3 months l year.Conclusions It is necessary to treat with surgery as early as possible if the amount of chyle drainage is more than 1 000 ml per day.
