抗β_2糖蛋白Ⅰ抗体与乙型肝炎后肝硬化细胞外基质的相关性研究

摘　　要：目的　研究乙型肝炎后肝硬化患者血清中抗 β2 糖蛋白I抗体 (aβ2 GPⅠ )与肝细胞外基质的相关性。方法　选择明确诊断的慢性乙型肝炎 (CHB)和乙型肝炎后肝硬化 (PHBC)患者为研究对象 ,以纯化的人血清β2 GPⅠ为抗原 ,用酶联免疫方法检测这些患者血清中的aβ2 GPⅠ水平 ,并且同时检测这些患者血清中的Ⅳ型胶原(Ⅳ -C)、透明质酸 (HA)和层粘连蛋白 (LN)。结果　发现PHBC患者血清中aβ2 GPⅠ水平与对照组比较有明显升高 (P <0 0 1) ,并发现PHBC患者血清中aβ2 GPⅠ高于CHB组 (P <0 0 5 )。同时发现PHBC患者血清中aβ2 GPⅠ的升高程度与肝功能分级无关 ,而与肝细胞外基质HA、Ⅳ -C、LN的血清水平呈明显正相关。结论　PHBC患者血清中aβ2 GPⅠ水平的升高 ,不仅与肝细胞受损有关 。Objective To study the relationship between anti-β 2-glycoprotein I(aβ 2GPⅠ) and extracellular matrix (ECM) in patients with post-hepatitis B cirrhosis (PHBC).Method The of patients with chronic hepatitis B (CHB) and PHBC were studied for the serum aβ 2GPⅠ levels by ELISA with purified β 2GPⅠ as antigen, and we investigated serum level of collagen IV (IV-C)?hyaluronic acid (HA) and laminin (LN) in these patients.Result High positive rate was observed compared with control group (P<0 01) and aβ 2GPⅠ level of PHBC group was higher than that of CHB group (P<0 05), and in patients with PHBC high serum levels of IV-C?HA and LN were found to be related to the elevated aβ 2GPⅠ level.Conclusion HBV infection may contribute to the generation of aβ 2GPⅠ. ECM in the setting of liver fibrolysis is another factor for aβ 2GPⅠ generator.
