皮损似传染性软疣的隐球菌病

摘　　要：目的提高对隐球菌病的诊断水平。方法对2例皮损似传染性软疣样的隐球菌病的临床资料及多项检查结果与文献进行对比分析。结果2例均为儿童,临床表现主要有发热、咳嗽、头痛、腹泻、肝脾肿大、关节痛和似传染性软疣的皮疹皮损组织液、血液及脑脊液检查找到隐球菌,皮损和淋巴结组织病理检查见孢子。结论当发热性疾病伴传染性软疣样皮损时,应警惕隐球菌感染。避免误诊和延误诊断的关键是提高对本病的认识和对可疑病例做真菌检查。ObjectiveTo improve the diagnosis of cryptococcosis. Methods2 cases of clinical data、 hemogram andmyelogram of cryptococcosis resembling molluscum contagiosum were analyzed. ResultsAll patients were children. The clinicalfeatures were consisted of fever、 cough、 headache、 diarrhea、 arthralgia、 hepatosplenomegaly and papules resembling molluscumcontagiosum. Cryptococcosis neoformans could be found in skin lesion, blood and CSF samples. The histological study of skin lesionand lymph node revealed numerous encapsulated budding yeast forms.ConclusionsThe febrile patients with skin lesions resem-bling molluscum contagiosum should be thought of cryptococcosis. The key points to avoid misdiagnosis and delayed diagnosis arerecognition earlier and laboratory identification of the fungus in suspected patients.
