冠心病伴左心室舒张功能不全应用曲美他嗪辅助治疗观察

摘　　要：目的 :评价曲美他嗪 (TMZ)与传统药物相结合治疗冠心病伴左心室舒张功能不全的临床疗效。方法 :78例冠心病伴左心室舒张功能不全患者随机分为治疗组和常规组 ,2组患者均予 β受体阻滞剂、钙拮抗剂、长效硝酸酯、转换酶抑制剂常规治疗。治疗组加用TMZ 2 0mg ,每天 3次 ,总疗程 48周。结果 :①常规组疗效总有效率 47 4% ,治疗组总有效率 80 0 % ,两组间比较有显著性差异 (P <0 0 5 )。②两组治疗后舒张早期血流峰值 (E)增加、心房收缩期血流峰值 (A)降低、舒张早期充盈减速 (DC)增加、E/A增加 ,提示两组治疗 48周后均可改善左心室舒张功能指标 ,与治疗前比均有显著性差异 (P <0 0 1～ 0 0 0 1) ,但治疗组各舒张功能指标改善更显著 ,与常规组比较亦有显著性差异 (P <0 0 0 1)。结论Objective: To investigate the effects of trimetazidine (TMZ) on the diastolic dysfunction in patients with coronary heart disease Methods: Seventy eight patients were divided randomly into control group (conventional therapy, n =38) and TMZ group (conventional therapy +TMZ, n =40) Diastolic function was examined before and after TMZ treatment (60 mg/d, for 48 weeks) Results: The total response rates were 47 4% in the control group and 80% in the TMZ group ( p <0 05) The left ventricular diaslotic function in both groups was improved There was significant difference before and after treatment ( p <0 01～0 001).In the TMZ group the parameters of left ventricular diastolic function was more remarkablely improved than those in the control group( p <0 001) Heart rate and blood pressure did not change significanthy ( p >0 05) during the treatment No adverse reactions were found Conclusion: TMZ combination can improve the diastolic dysfunction in patients with coronary heart disease It may be an adjuvant drug for the disease
