三尖瓣环的M型运动曲线和瓣环多普勒组织成像的运动频谱在下壁合并右室心肌梗死后评价右室功能中的作用

摘　　要：目的 :探讨对下壁合并右室心肌梗死后的患者 ,三尖瓣环的 M型运动曲线与瓣环多普勒组织成像(DTI)的运动频谱在评价右室功能中的价值。方法 :分别检测下壁合并右室心梗组 (A组 )、前壁心梗组 (B组 )和正常对照组 (C组 )的三尖瓣环的运动幅度、瓣环的收缩期峰值运动速度、舒张早期峰值运动速度 (E)与舒张晚期峰值运动速度 (L )。结果 :我们发现与 B、 C两组比较 ,A组的三尖瓣环运动幅度明显减小 (A组与 B组分别为 1.93与 2 .4 2 cm ,P<0 .0 1;A组与 C组分别为 1.93与 2 .6 8cm,P<0 .0 1) ;A组的瓣环收缩期峰值运动速度明显减小(A组与 B组分别为 11.3与 15 .2 cm/ s,P<0 .0 1;A组与 C组分别为 11.3与 15 .7cm / s,P<0 .0 1) ;同时 A组瓣环的舒张早期峰值运动速度亦减小 (A组与 B组分别为 9.1与 12 .6 cm/ s,P<0 .0 1;A组与 C组分别为 9.1与12 .7cm/ s,P<0 .0 1)。结论 :三尖瓣环的 M型运动曲线和瓣环的Objective:To evaluate the value of tricuspid annular motion and tricuspid annular velocity in Doppler tissue imaging (DTI) for assessing right ventricular (RV) function.Methods:The tricuspid annular motion was determined by M-mode echocardiography,and the tricuspid annular peak systolic, peak early (E) and peak late (L) diastolic velocities were determined by DTI in patients with inferior and RV myocardial infarction (MI) (group A), with anterior MI (group B) and without a history of heart disease (group C).Results:The tricuspid annular motion was reduced in group A compared with that in group B (1 93 vs 2 42 cm, P <0 01) and in group C (1 93 vs 2 68 cm, P <0 01) The peak systolic velocity of the tricuspid annulus was reduced in group A compared with that in group B (11 3 vs 15 2 cm/s, P <0 01) and in group C (11 3 vs 15 7 cm/s, P <0 01) The peak early diastolic velocity of the tricuspid annulus was reduced in group A compared with that in group B (9 1 vs 12 6 cm/s, P <0 01) and in group C (9 1 vs 12 7 cm/s, P <0 01).Conclusions:The results suggest that the tricuspid annular motion and the annular velocity by DTI can be used to assess RV function in patients with inferior and RV MI.
