清醒兔失血性休克后外周血白细胞数量和分类的变化

摘　　要：作者给兔放动脉血,使清醒兔平均动脉压降至6.67 kPa,维持1h,回输自体血和平衡盐液复苏.动态观察兔失血性休克后外周血白细胞数量和分类的变化.结果表明,兔失血性休克1h(再灌注前)外周血白细胞总数、淋巴细胞及中性粒细胞绝对计数均明显降低(P<0.01).复苏后第1,2日,白细胞总数由休克前的7.7±1.5×10~9/L升高到13.2±6.5×10~9/L(休克后1d,P<0.01)和11.2±3.7×10~9/L(休克后2d,P<0.05),中性粒细胞绝对计数由休克前的2.5±0.6×10~9/L增加到8.9±6.2×10~9/L(休克后1d,P<0.01)和4.9±2.1×10~9/L(休克后2d,P<0.05).淋巴细胞绝对计数在复苏后第1日仍显著低于休克前水平(3.4±1.6×10~9/Lvs5.2±1.2×10~9/L,P<0.05).单纯插管术后第1日白细胞总数和分类与术前没有显著差异.Waking rabbits were bled to achieve a mean arterial pressure of 6.67 kPa. The animals were maintained at this pressure level for one hour and then resuscitated with the shed blood and Ringer's lactate solution. Changes in WBC counts and classifications after shock were compared to the baseline values by means of Student's t test. One hour after shock, the total count of WBC, lymphocytes and neutrophils were all decreased (P<0.01). It was most likely due to blood loss and dilution. 24, 48 h after shock and resuscitation, the total count of leukocytes increased from 7.7±1.5×109/L (pre-shock) to 13.2±6.5 × 109/L (24 h post shock, P<0.01) and 11.2 ± 3.7 × 109/L (48 h after shock, P<0.05). The number of nutrophils also elevated from 2.5±0.6 × 109/L (pre-shock) to 8.9±6.2×109/L (24 h after shock,P<0.01) and 4.9±2.1 × 109/L (48 h after shock, P<0.05).The lymphocyte count was still depressed 24 h after shock (5.2±1.2×109/L before shock vs 3.4±1.6 × 109/L 24 h after shock, P<0.05). No significant changes in leukocytes were detected 24 h after catheterization.
