带骨间前动脉蒂的桡骨瓣移植治疗腕舟骨骨不连骨坏死

摘　　要：目的　探讨治疗腕舟骨骨不连骨坏死的手术方法。方法　对 13例腕舟骨骨不连、骨坏死的病人 ,采用以骨间前动脉背侧支为蒂的桡骨瓣逆行移植的手术方法治疗。结果　术后临床随访 3～ 2 8个月 ,腕部肿胀疼痛及无力症状均消失。 13例在 3～7个月内 ,X线片示骨折已全部骨性愈合及骨密度趋向正常。结论　以骨间前动脉背侧支为蒂的桡骨瓣逆行移植治疗腕舟骨骨不连骨坏死 ,具有血管解剖恒定、手术操作简单 ,术后疗效确切的优点 。Objective To probe into the operative way to treat scaphoid nonunion and its necrosis.Methods Thirteen patients with scaphoid nonunion were treated by transplantation of reverse radial bone graft with pedicel of dorsal branch of anterior interosseous artery.Results Their wrist swelling and disabling disappeared through followed-up observation for 3～28 months.Bony union and density tended to be normal in X-ray film postoperative 3～7 months.Conclusion The transplantation of reverse radial bone graft with pedicel of dorsal branch has the advantages:little variation of blood vessel anatomy,easy to operate and the good effect after operation.
