摘  要:【目的】 通过对佝偻病患儿骨钙素 (OC)、骨碱性磷酸酶 (BALP)、I型前胶原羧基端前肽 (PICP)和 2 5 羟维生素D3 [2 5 (OH)D3 ]的检测 ,观察其成骨细胞的功能状况 ,以便探讨合理的治疗原则。 【方法】 选择 40例维生素D缺乏性佝偻病患儿为受试对象 ,年龄为 6个月~ 1岁 ,男 18例 ,女 2 2例 ;取血后即注射维生素D3 3 0万IU ,4~ 5周后 ,复查者 18例 ;同时检测了年龄、性别相匹配的 2 0例健康婴儿作为正常对照 ,男 7例 ,女 13例。 【结果】 ①佝偻病患儿治疗前血清BALP、PICP与对照组相比显著升高 ;而OC、2 5 (OH)D3 与对照组相比显著降低。②给予维生素D3 4~ 5周后 ,与治疗前相比 ,BALP和PICP值明显下降 (P <0 .0 1) ;而OC和 2 5 (OH)D3 则明显升高 (P <0 .0 1)。③佝偻病患儿各指标之间的相关性 :2 5 (OH)D3 与OC呈正相关 (P <0 .0 1) ;OC与BALP呈负相关 (P <0 .0 1) ;BALP与PICP呈正相关 (P <0 .0 1)。 【结论】 作为成骨细胞的分子标志物OC、BALP和PICP ,可以反映佝偻病患儿的骨矿化程度。To observe the changes of the biochemical markers osteocalcin(OC), bone alkaline phosphatase (BALP),carboxyterminal of type I procollagen (PICP) and 25-hydroxyvitamin D_3[(25-(OH) D_3)], from of which can reflect the functions of osteoblast for children with vitamin D deficient rickets, and study the principle therapy principle for these patients. 40 children aged 6m~1y with vitamin D deficienty rickets were studied,18 of them were boys, and 22 of them were girls. All patients were treated with vitamin D_3 300,000 IU by intramuscular injection. 4 to 5 weeks later, 18 children were studied again. 20 healthy children were matched with sex and age as control.①Before treatment, serum levels of BALP and PICP were significantly higher than controls,and serum OC and 25-(OH)D_3 levels were significantly lower than controls.②After treatment, serum levels of BALP and PICP decreased ,whereas levels of OC and 25-(OH) D_3 increased .The changes of the four biochemical markers were all significant (P<0.01). ③The correlation between some markers of the patients: 25-(OH) D_3 correlated positively with OC, and the same relationship was found between PICP and BALP (P<0.01). However, OC showed negative correlation with BALP.[Conclusions]OC?BALP and PICP may act as the molecular markers of osteoblast, and reflect the degrees of bone minerlization for children with vitamin D deficient rickets.
