全国医院感染监控网医院感染肠球菌耐药性分析

摘　　要：目的 :了解肠球菌对常用抗菌药物的耐药特点 ,为临床治疗提供参考。方法 :对全国医院感染监控网 79家医院的医院感染肠球菌的来源及细菌耐药性进行统计分析。结果 :10 2 2株肠球菌占所有医院感染病原体的 4 .96 % ,其中为粪肠球菌 (5 2 .0 5 % )、屎肠球菌 (2 0 .2 5 % )、坚忍肠球菌 (8.0 2 % )。主要分离部位来自泌尿道 (30 .6 3% )、下呼吸道 (2 7.4 0 % )、手术部位 (15 .0 7% )。对青霉素、氨苄西林、呋喃妥因、万古霉素的耐药率较低 ,分别为 39.6 2 %、37.19%、2 2 .76 %、3.78%。结论 :肠球菌是引起医院感染的常见病原体 ,粪肠球菌多于其它肠球菌。不同感染部位分离的肠球菌种类有差异。肠球菌耐药严重 ,屎肠球菌的耐药性高于粪肠球菌。万古霉素、呋喃妥因、氨苄西林、青霉素对肠球菌敏感性较高。临床应针对不同情况合理选用抗菌药物。Objective:To realize the drug-resisting trait of enterococcus against common antibacterials to provide reference for clinical therapy.Methods:Resource and drug resistance of enterococcus in hospital infection in 79 hospitals that are members of National Monitoring Net of Hospital Infection was statistically analyzed.Results:1022 enterococci accounted for 4.96 % of all pathogens in hospital infection, consisted of E.faecalis (52.05 %), E.faecium (20.25%),E.durans(8.02%). The localities where enterococci were separated from were mainly urinary tracts (30.63 %), lower respiratory tracts (27.40 %) and operation sites (15.07 %). Enterococcus resisted many antibacterials with different resistance incidence, antibacterials of lower resistance inidence are penicillin (39.62 %), ampicillin(37.19 %), furazolidone (22.76 %) and vancomycin (3.78 %).Conclusion:Enterococcus was a common pathogen that brought hospital infection, and E.faecalis was more often found than other enterococcus. The kind of enterococcus varied in different localities. The drug resistance of enterococcus was serious, and drug resistance of E.faecium was stronger than E.faecalis. However, enterococcus was sensitive to vancomycin, furazolidone,ampicillin and penicillin. Antibacterials should be reasonably applied according to different clinical situation.
