摘  要:目的 探讨远红外热像图在腰椎间盘突出症的功能评定及疗效评定中的应用价值。方法 对 48例腰椎间盘突出症患者康复治疗前、后行远红外热像图检查 ,用目测类比评分法 (VAS)对疼痛进行评估 ,同时对症状、体征进行临床评定 ,分析腰椎间盘突出症患者的远红外热像图特点。结果 康复治疗后患者症状及体征均有明显改善 ,疗效优良率为 83 .3 % ,疼痛明显减轻 (P <0 .0 5 )。患者的远红外热像图表现为 :腰骶部出现异常热区 ,呈菱形或梭形 ,可呈现为片状均匀红色 ,多以L4~ 5及L5~S1 部位较突出 ,热区范围扩大 ;异常热区中心温度多超过 3 4℃ ,与周边温度差多超过 3~ 4℃。治疗后远红外热像图改善主要表现为 :热区范围减小和异常热区中心温度降低。结论 远红外热像图能直观反映腰椎间盘突出症异常热区的分布范围 ,可作为腰间盘突出症诊断及疗效评定的功能性指标。Objective To evaluate the value of far-infrared thermography in the functional evaluation and outcome assessment of patient with lumbar disc herniation. MethodsForty-eight cases of lumbar disc herniation were examined with far-infrared thermograph and visual analogue scale (VAS) before and after the rehabilitation treatment. The characteristics of far-infrared thermogram of patient with lumbar disc herniation were analysed in addition to the clinical assessment. ResultsThe symptoms and signs of the patients were improved after rehabilitation in 83.3% of the patients. The pain was relieved significantly (P< 0.05). The far-infrared thermogram of the patients presented as patch-shaped redness or rhombus-or shuttle-shaped abnormal hot section in the lumbosacral area. The temperature in center of the abnormal hot section exceeded 34℃ and the difference between the center and the margin areas exceeded 3~4℃. The improvement after the treatment was the reduce of size of the hot area and the temperature in center of the abnormal hot section. ConclusionThe far-infrared thermogram can reflect the distribution of the abnormal hot section in patient with lumbar disc herniation and be used as a functional criterion for the diagnosis and the evaluation of curative effect of lumbar disc herniation.
