摘  要:目的 :研制股骨颈穿针导航器 (FPN)并进行临床评价。方法 :用不锈钢材料制造一种可全方位调节股骨颈颈干角与前倾角的FPN ,应用FPN对股骨颈与股骨粗隆部骨折病例施术 46例 (高龄组 16例 ,导航组 3 0例 ) ,与对照组(3 0例 )做对比分析。结果 :导航组的术中股骨颈穿针次数与平均手术时间明显少于对照组 (P =0 .0 0 2~ 0 .0 19) ;导航组与高龄组的股骨颈植钉 (针 )满意率为 10 0 % ,明显高于对照组 (P <0 .0 5 ) ;高龄组手术安全、快捷 ,无 1例发生手术意外 ,术后卧床时间短 ,康复快。结论 :FPN可在股骨颈内全方位导航 ,定位准确、穿针成功率高 ,可缩短手术时间、减少手术创伤、提高手术疗效 ,适用于股骨颈与股骨粗隆部骨折的多种固定术式 ,尤其适用于高龄、体弱、合并症多、不宜行传统开放性手术的经皮穿刺多针内固定术。Objective:To develop femoral neck needle penetration navigator and carry out the clinical appraisement.Methods:The author made a needle penetration navigator of femoral neck which can regulate the neck-shaft angle and the pitch angle omni directional and treated 46 cases(16 cases in oldster group,30 cases in navigator group)with femoral neck fracture or intertrochanteric fracture of femur by operative with application of the navigator,compared the result with the control group(30 cases)and made a foresee analysis.Results:The times of needle penetration and average operative time in navigator group were evidently less than those in control group(P=0.02~0.019);The satisfactory rate of needle penetration in navigator group and lodster group is 100%.It was observably higher than that in control group(P<0.05);The operations in oldster group were safe and convenient without any accidence.The patients in oldster groups could spend less time on keeping in bed after operation and recovered more quickly.Conclusions:The navigator can be omni directional orient in the femoral neck exactly.The application of the navigator could shorten operative time,lessen the invasion,improve the healing affect.It is applicable to various operations for femoral neck fracture and intertrochanteric fracture of femur,sepecially to the percutaneous needle penetration for the old,weak patient and the patient with combined diseases.
