活力心浓缩水丸治疗扩张型心肌病临床研究

摘　　要：目的　观察自制活力心浓缩水丸对扩张型心肌病 (DCM)患者心功能、血流动力学、血浆内皮素及自身免疫水平的影响 ,并探讨其作用机制。方法　将 98例DCM患者随机分为治疗组 5 0例 ,对照组 48例。 2组均以西医常规治疗 ,治疗组加用活力心浓缩水丸治疗。比较 2组治疗前后心功能、血流动力学、血浆内皮素及免疫指标变化。结果　治疗组显效率、有效率、总有效率分别为 46.0 %、38.0 %和 84.0 % ,对照组分别为 33.3%、31 .3%和 64.6% ,2组比较有显著性差异 (P <0 .0 5 ) ;治疗组治疗后射血分数 (EF)、左室短轴缩短率 (ΔD % )和二尖瓣血流舒张早期流速 (VE) 左心房收缩期流速 (VA)均较对照组有显著性差异 (P <0 .0 5 ) ;治疗组治疗后血浆内皮素 (ET)、CD4、CD8、CD4 CD8、自然杀伤 (NK)细胞活性与对照组治疗后比较亦有显著性差异 (P <0 .0 5 )。结论　活力心浓缩丸能明显改善DCM患者的心功能和血流动力学 ,与西药合用能取得更好的治疗效果 。Objective To observe the effect of Huolixin concentrated water paste pill on the cardiac function, hemodynamics,plasma endothelin and autoimmune level in patients with dilated cardiomyopathy (DCM).Methods 98 patients with DCM were randomly divided into two groups.Among 50 patients in treatment group,heart function Ⅱ degree 9 cases,Ⅲ degree 37 cases and Ⅳ degree 4 cases; among 48 patients in control group,cardiac function Ⅱ degree 12 cases,Ⅲ degree 34 cases and Ⅳ degree 2 cases.The patients in both groups were treated with routine western medicine,while the patients in treatment group were traeted with Huolinxin water paste concentrated pill in addition.The cardiac function, hemodynamics, plasma endothelin and autoimmune level were observed before and after treatment in both groups.Results The obvious effective rate, effective rate and total effective rate in treatment group were 46.0 %,38.0 % and 84.0 % respectively,compared with those (33.3 %,31.3 % and 64.6 %) in control group (P<0.05);there were significant differences in plasma endoline,CD 4,CD 8,CD 4/CD 8,NK cells before and after treatment between the two groups (P<0.05).Conclusion Huolixin concentrated water paste pill can obviously improve the cardiac function,immunologic function and hemodynamics in patients with DCM,which will be more effective if it is used with western medicine together.
