摘  要:目的系统评价丙泊酚-芬太尼联合阿托品用于人工流产术(Induced abortion,IA)的有效性。方法系统检索CNKI、VIP、CBM、万方数据库、Pub Med、Embase、Cochrane Library数据库,同时手工检索相关杂志,纳入丙泊酚-芬太尼联合阿托品用于IA的随机对照试验,由两名研究员独立进行文献筛选、资料提取和方法学质量评价,并采用Cochrane系统评价方法和Rev Man5.3软件进行Meta分析。结果共纳入13篇文献1 348例患者,Meta分析显示:丙泊酚-芬太尼联合阿托品用于IA能减少麻醉过程中平均动脉压、收缩压、舒张压的下降,减弱对呼吸频率的抑制,且对患者心率、清醒时间和血氧饱和度无影响;由于相关报道较少,术后疼痛和恶心呕吐减少的结论仍需进一步验证。结论基于现有文献证据,丙泊酚-芬太尼联合阿托品用于人工流产术麻醉效果更好,但由于纳入研究方法学质量不高,统计学处理不当,有待于高质量临床试验进一步验证。Objective To systematically review the efficacy of propofol-fentanyl-atropine in induced abortion(IA). Methods Databases including CNKI, VIP, CBM, Wang Fang Data, Pub Med, Embase and Cochrane Library were electronically searched for the randomized controlled trails of using propofol-fentanyl-atropine in induced abortion. Two researchers independently screened literature, extracted data, and assessed the methodological quality. Then,according to the Cochrane methods, a meta-analysis was performed using Rev Man 5.3 software. Results A total of13 studies involving 1 348 patients were included. The meta-analysis showed that using propofol-fentanyl-atropine in IA can reduce the decrease of mean arterial pressure, systolic blood pressure, diastolic blood pressure, and the inhibition of respiratory frequency, with no effect on the patient’s heart rate, awake time and blood oxygen saturation. The decrease of the postoperative pain, nausea and vomiting still needs to be further verified because of lacking related reports. Conclusion Based on the current literature evidence, the efficacy of using propofol-fentanyl-atropine in induced abortion is better than using propofol-fentanylc in IA. Due to the low quality and improper statistical method of the included studies, double-blind randomized controlled trials are needed for further verification.
