摘 要:目的:探讨^(99)Tc-ECD在正常成人脑中的分布模式,这种分布在个体间的变异情况,在不同部位的分布与年龄、性别的关系以及目测分析与常用半定量分析方法的相关性。方法:对34例健康志愿者(年龄范围20~74岁,平均年龄35.03±11.81岁)行^(99)Tc-ECD SPECT脑血流灌注显像。选相应横断面图像分别在与左右小脑、下中额、下侧额、额颞部、颞枕部、下枕部、尾状核、壳核、中额部、中侧额、上额颞、上后颞、上枕部、距状裂、上中额、上侧额、感觉区、侧顶部及上距状裂相当的部位勾画38个感兴趣区(ROI),求出每个ROI内的每像素平均计数值。分别计算各ROI平均计数与双侧小脑平均计数比值和与脑皮质平均计数的比值。同时,采用7级评分法,分别由3位核医学医师对上述ROI部位的图像进行评分。然后,对各部位目测分析及半定量分析的数据进行统计比较。结果:^(99)Tc-ECD在脑不同部位中的分布差异较大,根据目测评分的结果,评分范围+3分~-1分。评为3分最多的部位依次为上距状裂、下距状裂和小脑,评为-1分最多的部位依次为壳核、上枕部、下枕部和颞枕部。但ECD在相同部位的个体间差异却较小,应用平均计数与小脑平均计数比值、平均计数与脑皮质总平均计数的比值及左右各对应部位平均计数的比值进行分析,其平均CV分别为9.06%、6.Objective: To elucidate the distribution pattern of 99mTc- ECD in brain of health adult with different age and sex and the correlation of visual and semi - quantitive analysis. Methods: 34 health volunteers were performed 99mTc - ECD SPECT brain perfusion imaging. The average age of these volunteers is 35.03111.81 (20 - 74 years old). After reconstruction of the transverse image, 38 ROIs were drawn in left and right cerebellum, inferior middle frontal, inferior lateral frontal, frontal temporal, occipital temporal, inferior occipital, caudate nucleus, putamen, middle frontal, middle lateral frontal, superior frontal temporal, superior posterior temporal, superior occipital, calcarine fissure, superior middle frontal, superior lateral frontal, sensory area, lateral parietal and superior calcarine fissure respectively. The average counts per pixel in each ROI were calculated. Then, we calculate the ratio of ROI average counts with the average counts of bilateral cerebellum and the average counts of the whole cerebella. In addition, 3 nuclear medicine physicians analyzed each ROI site score using a 7 grade score method. Results: The 99mTc-ECD distribution showed great difference in different site. The score in visual analysis ranged from + 3 to - 1. The site with most + 3 score were superior calcarine fissure, inferior calcarine fissure and cerebellum. The site with most - 1 score were putamen, superior occipital, inferior occipital and occipital temporal. But the difference was relatively small in the same site. The CV of the ratio of average ROI counts with average cerebellum counts and average whole cerebella and the ratio of bilateral corresponding area were 9.06% , 6. 59% and 6.28% respectively. The site, which had large variance, was basal ganglia. Correlation analysis of semi - quantitive and visual score showed significantly correlation of these two methods (r = 0.756, P< 0.001). we did not find variance of any site between male and female had significantly difference.
