摘　　要：目的　探讨V1 - 3导联心电图对急性后壁心肌梗死 (PAMI)的诊断价值。方法　对 36例PAMIV1 - 3导联进行观察分析 ,并与 1 0 0例正常人作对照。结果　PAMI时 ,V1 导联rs≥ 1、r波时限≥ 0 0 4s的特异性分别为 99%、86 % ,高于V2 导联 ,V2 导联两项指标敏感性为 72 2 %、88 9% ,高于V1 导联 ,V1 - 3导联ST段压低敏感性低(36 1 % ) ,特异性高 (1 0 0 % ) ,V1 - 3导联不仅表现T波高耸 ,且TV2 >TV1 -3 ,更为多见。结论　V1 - 3导联r/s≥ 1、r波时限≥ 0 0 4s、ST段压低、TV2 ≥TV1 -3 ,对PAMI有重要价值。Objective To explore the diagncstic value of changes in leads V 1-3 at posterior wa ll acu te myocardial infarction(PAMI).Methods ECG in lead V 1-3 of 36 cases with PAMI were analysed and compared other 100 nomal cases controls ed.Results Specificity of r/s≥1?r≥0 04 s leads V 1-3 (99%?86%) was higher than in leads V 2 after PAMI,but sensitilty in leads V 2 72 2%?88 9%),higher than in leads V 1,T wares in leads V 1-3 not only upright but also TV 2>TY 1-3 ,more frequently. Conclusion In lrads V 1-2 with r/s≥1,r≥0 04 s and in leads V 1-3 with ST segment depression,T V 2 >T V 1 a re important diagnostic valuse for PAMI.It has special cliniat significance buca use of routine leads to be used.
