可控性回结肠膀胱术后并发症的预防及处理(附8例报告)

摘　　要：目的 :预防可控性回结肠膀胱 (Indianapouch)术后并发症的发生 ,并提高对其治疗的水平。 方法 :总结我院 8例Indianapouch术后并发症的治疗经验及 2 2例Indianapouch尿流改道的手术经验。 结果 :8例并发症中 ,7例治疗结果满意 (87.5 % ) ,1例拒绝再次手术治疗。手术方式改良前期的 9例中 ,6例出现并发症(6 6 .7% ) ;改良后期的 13例中 ,仅 2例出现并发症 (15 .4 % )。并发症的发生率明显降低 (χ2 =6 .0 4 ,P <0 .0 5 )。结论 :Indianapouch术后的大多数并发症是可以治愈的 ,部分并发症可通过腔内镜微创手术治疗。随着手术技术的熟练和术式的改进 ,并发症的发生率明显降低。Purpose:To enhance the therapeutic level of Indiana pouch complications.Methods:The therapeutic expertence of surgical camplications in 8 cases of Indiana poach and the surgical experience in 22 cases of Indiana pouch were reviewed from 1998.1 to 2001.12.Results:The results were satisfactory in 7( 87.5 %), 1 case refused to operate again.Before surgical modification, 6 cases presented the complications in 9( 66.7 %), after that, only 2 cases in 13( 15.4 %). The complications reduced significantly (χ 2 = 6.04 ,P< 0.05 ).Conclusions:Most of the complications of indiana pouch could be cured, part of which may be treated by endoscopy. Complications of indiana pouch reduced with skilled techniques and modification.
