帕瑞昔布钠在无痛宫腔镜检查的应用

摘　　要：目的:探讨帕瑞昔布钠复合异丙酚在无痛宫腔镜检查中的麻醉效果。方法:选择我院行宫腔镜检查的妇科患者180例随机数字表随机分成C、F、P三组各60例,C组单纯使用异丙酚,F组使用芬太尼+异丙酚,P组使用帕瑞昔布钠+异丙酚,分别观察三组患者在麻醉诱导前、给药后3min、手术结束时的心率、平均动脉血压、SpO2等指标变化,以及异丙酚用量、意识消失时间、苏醒时间、离院时间、不良反应的发生情况。结果:C组和F组心率、平均动脉血压、SpO2在给药3min时明显低于麻醉诱导前;C组苏醒时间、异丙酚用量明显高于F组和P组;C组术后宫缩痛明显高于F组和P组;F组头晕发生率明显高于C组和P组,P组呼吸抑制明显低于C组和F组。结论:帕瑞昔布钠复合异丙酚用于无痛宫腔镜检查术,镇痛效果好,无明显不良反应,是无痛宫腔镜检查术又一理想麻醉方法,可用于临床。Objective To discuss the anesthetic efective of parecoxib for painless hysteroscopy.Methods 180 ASA class I-II patients who would undergo painless hysteroscopy were randomly assigned to three groups(C,F,P group) by stable of random number,60 patients each group,the group C was only propofol; the group F was propofol combined with fentanyl and the group P was propofol combined with parecoxib.The heart rate,Mean arterial blood pressure,oxygen saturation of pulse(SpO2) were recorded among 3 groups at three time points.The dosage of propofol,consciousness loss time,recovery time,hospital stay time and the incidence of adverse reactions were observated.Results The heart rate(HR),mean arterial blood pressure(MBP) and SpO2 of the group C and F in 3 minutes after administration was significantly lower than before anesthesia.the dosage of propofol and recovery time of group C were significantly higher than that group F and group P; the postoperative uterine contraction pain of group C was higher than that the F group and P group; the incidence of dizziness was obviously high in group F than that in group C and group P,Prespiratory inhibition of the group P was lower than that in group C and group F.Conclusion Parecoxib combined with propofol for painless hysteroscopy had good anesthetic efective,and no significant adverse reaction,it is a ideal anesthesia method for painless hysteroscopy,and it’s worth for clinical application.
