摘　　要：目的　探讨慢性阻塞性肺疾病患者 (COPD)吸入糖皮质激素对气道炎症的影响。方法　测定了 19例 COPD稳定期患者吸入糖皮质激素前后不同时间痰中性粒细胞、白细胞介素 - 8(IL - 8)及肺功能的变化 ,并与健康人进行比较。结果　 11例完成治疗。 COPD稳定期患者治疗前痰细胞总数、中性粒细胞百分比及 IL - 8水平均明显高于健康对照组 (p<0 .0 1) ,治疗 3月后痰 IL - 8水平较前明显降低 (p<0 .0 5 ) ,肺功能不同时间亦有明显改善 (p<0 .0 5 )。结论　 COPD稳定期吸入糖皮质激素对改善肺功能。Objective To assess the effects of inhaling corticosteroids on airway inflammation among patients with stable COPD. Methods The percentages of neutrophils, IL 8 level of the sputum and the lung function in 19 patients with stable COPD were measured before and after the treatment using inhaled corticosteroids. The treatment group was compared to 12 healthy subjects.Results A total of 11 patients completed the study.The total cell count, the percentage of neutrophils, and IL 8 level in the sputum of patients with COPD before the treatment was found higher than that of healthy subjects (p <0 .01 in each comparison). A 3 month treatment significantly reduced the IL 8 level of sputum among COPD patients (p < 0.05) but had no effect on total cell count and percentage of neutrophils. The lung function of the COPD patients apparently improved after the treatment (p < 0.05).Conclusion For the patients with stable COPD, use of inhaled corticosteroids may improve their lung function and decrease the level of airway inflammation. However, long term effects of the inhaled corticosteroids remain to be studied.
