血吸虫病中度流行区化疗对象筛查方案的研究

摘　　要：目的　探讨血吸虫病中度流行区简便、有效、经济、适用的化疗对象筛查方案。方法　分别以粪检、询检、间接血凝法 (IHA)及胶体染料试纸条法 (CDSIA,目前改称 DDIA)对居民进行血吸虫病筛查 ,并对各方法进行分析和评价。结果　经比较分析 ,IHA法在一些指标上优于其他方法 ,但每检出 1例病人的费用也高于其他方法。结合流行病学资料 ,本研究提出 3种 [A、B、C(包括 a,b两种情况 ) ]综合筛查方案 ,敏感性分别为 88.46%、80 .77%、82 .0 5 %和 92 .3 1% ;每检出 1例感染者所需费用分别为 8.5 3元、6.5 9元、8.78和 5 .97元。结论　 IHA经检验是一种简便、有效 ,在感染率为5 % -10 %的中度流行区较可取的筛查方法 ,但所需费用仍较高 ;DSIA较 IHA更为简便、快速、廉价 ,且敏感性高。各方法综合应用可提高检出率、降低成本。综合筛查方案 (Cb)具有减少工作量 ,降低漏检率 ,简便、经济 。Objective To develop simple, effective, low cost and valid screening methods for determining the chemotherapy subjects, which are applicable to the areas with moderate endemic of schistosomiasis.Methods The residents were screened respectively with inquiry examination, Kato Katz, IHA and CDSIA (now called DDIA) techniques. The results of examinations and the cost effectiveness were assessed. Results Some appraisal indices of IHA were better than that of others, but the screening cost of IHA was higher than that of the other methods. Three screening schemes were brought forward. They are A, B, Ca, Cb, and the sensitivities were 88 46%,80 77%,82 05% & 92 31%, respectively; the screening costs for one patient were 8 53 yuan,6 59 yuan,8 78 yuan & 5 97 yuan. Conclusion IHA technique was confirmed to be a kind of better screening method, but its cost was higher than that of the other methods. DDIA was also a good technique. It was simpler, less expensive, more rapid and sensitive. The Cb screening scheme is better than others used in the field.
