急性冠脉综合征患者血浆ET和TXA_2含量的变化及其意义

摘　　要：目的　探讨内皮素 (ET)和血栓素 (TXA2 )在急性冠脉综合征中血浓度的变化及其临床意义。方法　用放射免疫法测定 13例不稳定型心绞痛 (UAP)和 18例急性心肌梗死 (AMI)患者及 2 6例健康受试者血浆ET、TXA2 浓度。结果　UAP症状发作前后血浆ET和TXA2 分别为 (180 .2± 12 .1)ng·L-1vs(94.8± 2 0 .5 )ng·L-1和 (2 78.9± 14 .9)ng·L-1vs(14 5 .7± 15 .1)ng·L-1。AMI溶栓前后ET和TXA2 分别为 (3 44.2± 19.0 )ng·L-1vs(12 8.5± 17.0 )ng·L-1和 (4 5 2 .1± 12 .5 )ng·L-1vs(2 85 .2± 15 .5 )ng·L-1。对照组ET和TXA2 分别为 (4 3 .6± 4.7)ng·L-1和 (71.7± 11.9)ng·L-1。梗死面积与ET、TXA2 浓度呈正相关。结论　UAP症状发作前后 ,AMI溶栓前后ET和TXA2 有显著差异 ,ET和TXA2 浓度越高 ,心肌梗死面积越大。Objective To study the change in content of plasma endothelin (ET) and thromboxane (TXA 2) and its clinical significance in patients with acute coronary syndrome.Method The plasma levels of ET and TXA 2 were measured by radioimmunoassay in 13 patients with unstable angina pectoris (UAP) and 18 patients with acute myocardial infarction (AMI)and 26 healthy subjects.Results Before and after attack in patients with UAP,the plasma levels of ET and TXA 2 were respectively(180.2±12.1)ng·L -1 vs (94.8±20.5)ng·L -1 and(278.9± 14.9 )ng·L -1 vs (145.7±15.1)ng·L -1 .Before and after thrombolysis for AMI,ET and TXA 2 were respectively(344.2±19.0)ng·L -1 vs ( 128.5± 17.0)ng·L -1 and (452.1±12.5)ng·L -1 vs (285.2±15.5)ng·L -1 .The plasma levels of ET and TXA 2 in control group were respectively(43.6±4.7)ng·L -1 and(71.7±11.9)ng·L -1 .The area of myocardial infarct was positively correlated with the levels of ET and TXA 2.Conclusions There are significant differences in the plasma levels of ET and TXA 2 between before and after attacking of UAP,and between before and after thrombolysis in AMI.The higher levels of ET and TXA 2,the larger area of myocardial infarct.
