多胎妊娠经阴道B超引导下单纯胚胎抽吸减胎术32例分析

摘　　要：目的 :探讨经阴道B超引导下单纯胚胎抽吸减胎术的临床应用价值。方法 :对 1999年 12月至2 0 0 2年 10月体外受精 胚胎移植 (IVF ET)、单精子卵浆内注射 (ICSI)、诱导排卵后宫腔内人工受精 (IUI)、冷冻胚胎移植后妊娠 7～ 8w多胎妊娠 32例 (1例四胞胎、31例三胞胎 ) ,经阴道B超引导下将 17G穿刺针靠近或进入胚胎心管搏动处 ,利用负压抽吸胚胎。结果 :一次减 2胎 2例 ,一次减 1胎 30例。 2例分别于孕 11w和孕 17w流产 ,8例早产 (32～ 36w) ,16例足月产 (37～ 39w) ,6例在继续妊娠中。 2例出生单胎 ,2 2例分娩双胎 ,新生儿平均体重 2 32 6g(10 5 0～ 375 0g) ,出生的 4 6个小孩未发现外表畸形。结论 :经阴道B超引导下单纯胚胎抽吸减胎术操作简便、安全 。Objective:To evaluate the efficacy of early transvaginal embryo aspiration for selective reduction of multifetal pregnancies and its impact on the outcome of pregnancy.Methods:Thirty two cases of multifetal pregnancies underwent early (mean 7.5 weeks gestation, range 7.0～8.0 weeks) transvaginal embryo aspiration for selective fetal reduction. A 17G needle was inserted through the vaginal fornix and the uterine wall and directed into fetal chest where aspiration was performed.Results:The transvaginal embryo aspiration for reduction of two embryos was performed in two cases. 30 cases underwent aspiration procedures for reduction of one embryo. Two gestations miscarried at 11 and 17weeks respectively. Eight cases deliveried premature babies (32～36 weeks). Sixteen women delivered between 37 and 39 gestational weeks. Six women are ongoing pregnancy. Conclusion:Multifetal pregnancy reduction by transvaginal embryo aspiration is a safe and efficient method for improving outcomes of multifetal pregnancy.
