摘 要:目的 :探讨CD3单抗 (mAb)和淋巴细胞功能相关抗原 1单抗 (LFA 1mAb)对狼疮肾炎 (LN)患者外周血单个核细胞 (PBMC)的共刺激作用。 方法 :2 9例LN患者被分为活动期 (n =14)和非活动期 (n =15 ) ,以 12例健康献血员为对照 ,观察CD3mAb或 (和 )LFA 1mAb共刺激及阻断 1 磷酯酰肌醇 3 激酶 (PI3 K)对体外培养 96h后PBMC增生和IL 2产生的影响。PBMC提取采用Ficoll密度梯度离心法 ,细胞增生实验采用3H TdR掺入法 ,IL 2测定采用ELISA法。 结果 :PBMC培养 96h后 ,自然生长的LN非活动期和活动期PBMC3H TdR掺入量和IL 2合成与正常对照无明显差异 (P值均 >0或 0 0 5 ) ;与自然生长的PBMC相比 ,CD3mAb单独处理增加了LN非活动期 (P值均 <0 0 5 )和活动期 (P值均 <0 0 5 )PBMC3H TdR掺入量和IL 2合成。而单独CD3mAb对正常对照PBMC3H TdR掺入量和IL 2合成没有影响 (P值均 >0 0 5 )。与CD3mAb单独处理组相比 ,CD3mAb和LFA 1mAb共刺激均增加了LN活动期 (P值均 <0 0 5 )和非活动期 (P值均 <0 0 5 )及正常对照 (P值均 <0 0 5 )PBMC3H TdR掺入量和IL 2合成 ;与对照组相比 ,CD3mAb和LFA 1mAb共刺激后活动期 (P值均 <0 0 1)和非活动期 (P值均 <0 0 1)PBMC3H TdR掺入量和IL 2合成均增加 ,但活动期效应明显强于非活?Objective:To investigate the costimulating effect of CD3mAb and leukocyte function associated antigen 1 monoclonal antibody (LFA 1mAb) on proliferation and IL 2 production of peripheral blood mononuclear cells (PBMC) in patients with lupus nephritis (LN). Methodology:Twenty nine patients with LN (14 cases in the active stage and 15 in the inactive stage of LN) were selected for this study,and 12 healthy persons were served as controls.PBMC were treated with CD3mAb or/and FLA 1mAb and Wortmannin (WT).one of specific inhibitor of 1 phosphatidylinositol 3 kinase (PI3 K) for 96 hours.PBMC were obtained by Ficoll Density Gradient Centrifugation,and cell proliferation was detected by 3H TdR incorporation,and ELISA was used to measure IL 2 in supernatant of cultural PBMC. Results:After PBMC were cultured for 96h,in conspontaneous condition,there were no significance in 3H TdR incorporation and IL 2 contents of cultured PBMC among active LN (both of P >0 05),inactive (both of P >0 05) and controls(both of P >0 05),When PBMC from controls,active and inactive LN cultured with CD3mAb alone,compared with conspontaneous condition,the 3H TdR incorporation and contents of IL 2 in PBMC were increased in active LN (both of P <0 05) and inactive LN (both of P <0 05),but were not in healthy controls (both of P >0 05).Compared with CD3mAb alone,costimulation of CD3mAb and LFA 1mAbs enhanced markedly the 3H TdR incorporation and contents of IL 2 in PBMC in active LN (both of P <0 01,respectively),inactive LN (both of P <0 05,respectively),especially in active LN.WT markedly inhibited the contents of IL 2 and 3H TdR incorporation induced by LFA 1 costimulation in PBMC in LN (both of P <0 01,respectively). Conclusion:CD3 and LFA 1 has a costimulating effect on PBMC in LN,which can involve in abnormal activation of T and B cell.Wortmannin can inhibted the costimulation induced by CD3mAb and LFA 1mAb in PBMC in patients with LN.
