摘 要:目的 应用99mTcO4 -及99mTc -MIBI显像对功能自主性甲状腺瘤以外正常甲状腺组织影像进行临床评价 .方法 对 3 0例单发热结节病人同时分别应用99mTcO4 -及99mTc -MIBI显像 ,并测定高灵敏度TSH、FT3、FT4;2 3例行手术切除与之比较 .结果 99mTc -MIBI被甲状腺组织摄取 ,不受血中甲状腺激素浓度的影响 ,在“热”结节存在时 ,正常甲状腺组织仍摄取99mTc -MIBI .结节外正常组织显影 2 5例 ,占 83 3 % ,与应用99mTcO4 -甲状腺显像“热”结节屏蔽后正常甲状腺显影 2 6例 ,占 86 3 % .相比p >0 0 5 ,无明显差异 ,2 3例甲状腺“热”结节手术半年后行甲状腺显像 ,余留的甲状腺影象与治疗前显像相似 .结论 以上两种方法能使功能受抑制的正常甲状腺组织显影 ,能帮助鉴别诊断甲状腺结节的功能自主性 ,具有简便。Objective To appraise the evaluation of imaging in normal thyroid gland around the autonomic functional thyrophyma with 99m TcO - 4 and 99m Tc-MIBI. Methods 30 patients with single hot thyroidnodule had been imaged by 99m TcO- and 99m Tc-MIBI separately.Results Both normal thyroid tissues with and without hot nodule in it could absorb 99m Tc-MIBI and have not been affected by thyroxin concentration in the blood. The imaging ratio of 99m Tc-MIBI is 83 3%(25/30) in the normal tissue around the hot nodule of 30 patients. The imaging ratio of 99m TcO -is 86 3%(26/30) in the normal tissue around hot nodule after the hot nodule shielded in the 30 patients. There is no statistical difference between these two methods (p>0 05). Conclusion The two above methods can image the normal thyroid gland that the normal function has been repressed, and these methods are safety, simple and convenient for differential diagnosis of autonomic functional thyroid nodule. It could be used as a regular method substituting the TSH stimulating trial for thyroid imaging.
