特发性阵发性心房颤动与左室舒张功能关系探讨

摘　　要：目的 :探讨特发性阵发性心房颤动 (IPAF)的发生与左房左室结构、功能变化的关系。方法 :IPAF患者 2 2例 (IPAF组 ) ,健康对照 2 2例 (N组 ) ,两者均在窦性心律下用HP -SONOS 5 0 0 0型彩色脉冲多谱勒超声显像仪测量收缩末期左方前后径 (LAD) ,舒张末期室间隔及左室后壁厚度 (IVST ,LPWT) ,舒张末期左室内径 (LVEDD) ,二尖瓣舒张早期左室充盈速度峰值E(E峰 ) ,二尖瓣舒张晚期左室充盈速度峰值A(A峰 ) ,E/A比值及左室射血分数 (LVEF) ,计算左室重量指数 (LVMI)。结果 :N组与IPAF组的左房前后径 ( 2 .96± 0 .2 0 ;3.2 3± 0 .2 9P<0 .0 1 ) ;左室重量指数 (LVMI) ( 80 .2 9± 1 9.1 6;90 .90± 1 4.69P<0 .0 5) ,E/A比值 ( 1 .2 5± 0 .39;0 .965± 0 .2 9,P<0 .0 1 )均有显著性差异 ,而两组的左室射血分数、左室内径无统计学差异。结论 :特发性阵发性心房颤动的发生 (IPAF)与左房前后径 (LAD)增大 ,左室舒张功能减退及左室重量指数增加有关。Objective:To evaluate left ventricular structure and filling as well as left atrial dimension in patients with idiopathic paroxysmal atrial fibrillation(IPAF).Methods:left ventricular two-dimensional and M-mode echocardiograms and pulsed Doppler echocardiography of transmitral flow were performed in 22 consecutive patients with idiopathic paroxysmal atrial fibrillation and 22 normal subjects.Groups were matched for age and gender.Results:M-mode left atrial anteroposterior diameter (LAD) was greater in group IPAF than in group N.(group N 2.96±0.20,group IPAF 3.23±0.29,P<0.01),left ventricular mass index was higher in group IPAF than in group N(group N 80.29±19.16,group IPAF 90.90±14.69,P<0.05).The pulsed Doppler diastolic early/late velocity ratio(E/A) was lower in group IPAF than in group N(group N 1.25±0.39,group IPAF 0.965±0.29,P<0.01).Conclusion:These results suggest that the occurrence of idiopathic paroxysmal atrial fibrillation(IPAF) is associated with increased letf atrial dimension(LAD),increased left ventricular mass index(LVMI)and reduced diastolic early/late velocity ratio(E/A).
