慢性乙型肝炎肝组织学分级与血清HBV DNA及HBeAg定量的关系

摘　　要：目的　 探讨慢性乙型肝炎肝内炎性变化与HBVDNA定量及HBeAg定量的关系 ,为临床正确判断病期和治疗提供依据。方法　 应用荧光定量PCR法和放免法 ,分别对 10 2例慢性乙肝患者血清中HBVDNA和HBeAg含量进行定量检测 ,同时采用常规 1s肝穿刺活检法行肝穿病理组织学检查。结果　10 2例乙型肝炎患者肝内炎症组织学分级与HBVDNA定量无相关性。 4 5例HBeAb阳性患者的肝内炎症组织学分级与HBVDNA定量呈明显的相关。 5 7例HBeAg阳性患者肝内炎症组织学分级与HBVDNA无明显的相关 ,但有降低的趋势 ,肝内炎症组织学分级与HBeAg定量显著相关。结论　 在非免疫耐受期的慢性乙肝患者 ,血清HBVDNA复制水平与肝内炎症活动程度相关 ,肝内炎症患者与血清HBeAg含量密切相关 ,提示检测慢性乙型肝炎患者血清HBVObjective To explore the correlation between the intrahepatic inflammatory changes in patients of chronic hepatitis B(HB) and the levels of HBV,DNA and HBeAg so as to provide reliable judgement of disease stages and appropriate treatments.Methods 102 cases of chronic HB received the content detection of HBV,DNA and HBeAg with fluorescent PCR and RIA and the histopathological detections through puncture biopsy were simultaneously made.Results No correlation was found between the intrahepatic inflammtory histological grading and the contents of HBV and DNA in 120 cases of HB.Obvious correlation existed between the intrahepatic inflammatory histological grading of 45 cases of positive HBeAg and the contents of HBV and DNA,while no obvious correlations were found in other 57 cases,and the grading tended to decline among then.Obvious correlation existed between the intrahepatic inflammatory histological grading and the contents of HBeAg in those 57 cases.Conclusion The replication levels of HBV and DNA are closely related to the degrees of intrahepatic inflammations,and the intrahepatic inflammation is closely related to the content of HBeAg.The detection of HBV,DNA and HBeAg can help to choose the right treatment chances and to promote the curative effects on chronic HB.
