保肝丸治疗慢性乙型肝炎的临床观察

摘　　要：目的 :观察纯中药制剂保肝丸治疗慢性乙型肝炎的临床疗效。方法 :将 36 4例慢性乙型肝炎患者 ,随机分为治疗组与对照组 ,治疗组 184例采用综合保肝治疗 ,并服用保肝丸 ,对照组 180例予综合保肝治疗 ,并服用灭澳灵 ,观察保肝丸治疗慢性乙型肝炎 3个月后患者临床症状、体征、肝功能、肝纤维化指标以及乙型肝炎血清转换率的改善情况。结果 :治疗后两组肝功能除ALT差异无显著性外 ,其他项目的下降两组差异均有显著性 (P <0 0 5 )。保肝丸治疗后Ⅲ型前胶原 (PCⅢ )、层粘蛋白 (LN)、透明质酸 (HA)较治疗前显著降低 (P <0 0 1) ,与对照组比较差异亦有显著性 (P <0 0 1)。对肝脾的回缩与B超检查的好转有明显改善作用。结论 :保肝丸在抗肝纤维化与肝脾的回缩与B超检查的好转方面作用突出 ,为治疗慢性乙型肝炎较理想的药物 ,有改善和抑制病毒复制的作用。Objective: To observe the clinical effect of Baogan Pill (BGP, a pure Chinese herbal preparation) in treating chronic hepatitis B (CHB). Methods: Three hundred and sixty four patients with CHB were randomly divided into the treated group (n=184) and the control group (n=180), both groups were treated with comprehensive treatment for liver protecting, to the former group, BGP was added and to the latter group, conventional liver protection treatment and Mie′aoling was added. Changes of clinical symptoms, signs, liver function, anti fibrosis parameters and serum CHB conversion rates were observed. Results: After treatment, the various parameters of liver function except alanine transaminase (ALT) in the treated group, were all alleviated significantly (P<0 05), and the liver fibrosis indexes, procollagen Ⅲ (PCⅢ), laminin (LN) and hyaluronic acid (HA), were reduced significantly as compared with those before treatment (P<0 01),and also showed significant difference in comparing with those in the control group (P<0 05). Markedly improvement was shown in the treated group especially in the volume reducing of liver and spleen, improving of B ultrasonogram and parameters of early liver fibrosis. Conclusion: BGP, showing prominent effect in volume reducing of liver and spleen as well as improving of B ultrasonogram, is an ideal drug for treatment of CHB, it also could inhibit the replication of virus.
