摘  要:目的 观察地塞米松对兔内毒素性肺损伤期间胃粘膜 pH值 ( pHi)的影响。 方法 采用间隔24h注射内毒素 (LPS)制作家兔内毒素性肺损伤模型。成年雄性家兔静脉注射3μg/kgLPS ,24h后在机械通气下用微泵缓慢输注50μg/kgLPS ,待动态肺顺应性(Cdyn)下降到75%时分为两组 :治疗组 (n=8)静脉注射地塞米松1mg/kg;对照组 (n=8)静脉注射与治疗组相同容量的0.9 %氯化钠注射液。观察不同时点pHi、胃粘膜局部二氧化碳分压 (PrCO2)、胃粘膜 -动脉局部二氧化碳分压差 (Pr-aCO2)。将兔在地塞米松治疗后6h处死作形态学检查。结果pHi治疗组较对照组有明显升高 (P<0.01) ,而PrCO2 和Pr-aCO 2 值治疗组与对照组相比明显下降 (P<0.01)。形态学观察提示地塞米松可明显减轻肺损伤。结论 地塞米松明显减轻肺损伤可能与其明显提高内毒素性肺损伤期间 pHi有关。Objective To study the effect of dexamethasone on gastric intramucosal pH during acute lung injury induced by lipopolysaccharide in rabbits. Methods The model was prepared by intravenous injection of 3μg/kg of lipopolysaccharide followed by infusion of 50μg/kg of lipopolysaccharide after 24h. While pulmonary dynamic compliance (Cdyn) decreased to 75 percent of the baseline level, rabbits were randomly allocated into two groups: treatment group and control group with 8 animals in each. Dexamethasone (1 mg/kg) was administered intravenously in treatment group, while same volume of 0.9% NaCl was administered in control group. Gastric intramucosal pH, gastric intramural PCO 2 , the PCO 2 gap (intramural PCO2-PaCO 2) were measured before and after LPS infusion. Six hours after administration of dexamethasone, the animals were sacrificed. Lung tissue was also used to histological examination.Results pHi, PrCO2 and Pr-aCO 2 were improved in treatment group, compared with those in control group (P<0.01). In control group, the injury and inflammatory cell infiltration in the pulmonary stroma and alveoli were much severe than those in treatment group. Conclusion Dexamethasone could significantly alleviate gastric intramucosal pH changes induced by lipopolysaccharide, to reduce the severity of acute lung injury.
