摘 要:目的 研究先天性肥厚性幽门狭窄 (CHPS)幽门卡哈尔间质细胞发育和突触形成情况。方法 使用SP免疫组织化学技术对 32例CHPS和 1 0例正常对照者进行检测。结果 对照组 ,c kit抗体阳性细胞大量分布于肌间神经丛神经节细胞周围 ,中等量分布于环肌层 ,少量分布于纵肌层 ,它们相互联结形成网状 ;肌间神经丛内可见大量突触素抗体表达 ,纵肌和环肌层可见中等量突触素抗体表达。CHPS组 ,c kit抗体阳性细胞在肌间神经丛和环肌层明显减少 ,它们形成的网络被破坏 ;肌层突触素抗体表达显著减少 ,而肌间神经丛无明显变化。结论 CHPS有卡哈尔间质细胞发育异常和突触形成异常 。Objective To evaluate the role of interstitial cells of Cajal and synapse formation in congenital hypertrophic pyloric stenosis(CHPS). Methods The distribution of interstitial cells of Cajal and the synapses were examined in 32 cases of CHPS and 10 normal controls (including two neonates and eight adults) by SP immunohistochemistry with rabbit antihuman polyclonal c kit antibody and rabbit antihuman polyclonal synapse antibody.Results In the control group, there was no apparent difference in the number and distribution of c kit positive cells and synapse expressions between neonates and adults. There were abundant c kit positive cells around the myenteric plexuses, highlighting the plexuses. Moderate number of c kit positive cells were observed on the circular muscularis and only a few seen in the longitudinal muscularis. Under light microscopy(400×), c kit cells were characterized by small cell body,large nuclei, scanty perinuclear cytoplasm and a tendency for network formation. There were abundant synapse expressions in the myenteric plexuses and moderate expressions in the circular and the longitudinal muscularis. In the group of CHPS, c kit positive cells were absent in the myenteric plexuses. C kit positive cells apparently decreased in the circular muscularis and were seen occasionally in the inner part of the circular muscularis near the submucosal margin and the proximal part of the specimens. Positive cells were also reduced in the longitudinal muscularis. The network was destroyed.Conclusion Interstitial cell of Cajal and synapses may be a possible cause of congenital hypertrophic pyloric stenosis.
