摘 要:目的 :探讨炎症细胞因子在新生儿窒息后肾损伤中的作用。方法 :采用酶联免疫吸附 (EL ISA)法检测窒息患儿尿中白介素 8(IL 8)、IL 6、肿瘤坏死因子 α(TNFα)水平 ,并同时测定尿中反应肾小管损伤的指标 β2 微球蛋白 (β2 MG)、N 乙酰 β D 氨基葡萄糖苷酶 (NAG)和 γ 谷胺酰转肽酶 (γ GT)水平。结果 :IL 8、IL 6、TNFα、β2 MG、NAG、γ GT在轻度窒息组和重度窒息组较正常对照组以及重度窒息组较轻度窒息组均显著升高 (P均 <0 .0 5 ) ;在窒息组内 ,尿中各炎症因子水平与肾小管各损伤指标之间均呈显著正相关关系 (P均 <0 .0 5 )。结论 :窒息可诱导机体发生全身炎症反应综合征 ,进而导致窒息后肾损伤的发生。尿中炎症细胞因子水平的测定是判断窒息程度及肾损伤的重要指标。Objective:To explore the relationship between amount of inflammatory cytokines in urine and neonatal postasphyxial renal tubules injury.Methods:The level of inflammatory cytokines such as interleukin (IL8,IL6),tumor necrosis factorα(TNFα) and the indicators of evaluating renal tubules injury 〔Nacetylglucosaminidase(NAG), γglutamyltranspeptidase(γGT),β 2microglobulin(β 2MG)〕 in urine were detected in neonates with asphyxia.Results:Compared with control,the levels of IL8,IL6, TNFα and NAG,γGT,β 2MG were obviously increased in mild asphyxia group.In severe asphyxia group , the parameters above were all significantly increased compared with mild asphyxia group and the control group. Within the asphyxia group,there were positive relationship between inflammatory cytokines and the indicator of evaluating renal tubules injury.Conclusions:The asphyxia may induce systemic inflammatory response syndrome(SIRS),which result in postasphyxial renal injury in neonates.The level of inflammatory cytokines in urine may be used as the indicators of evaluating the severity of asphyxia and postasphyxial renal injury in neonates.
