摘　　要：评价VDD起搏器远期心房感知功能的可靠性。 1 4例VDD起搏器病人 ,术后一周内每天记录心电图 ,术后32 .50± 1 3 .2 0个月时行Holter检查 ,检测过程中 ,让病人保持各种不同体位、做Valsalva动作、深呼吸、咳嗽及运动各2min ,并记录相应时间 ,手工检测 2 4h心房感知率。结果 :术后一周内 1 2例心房电极感知功能正常 ,1例于术后第 3天发生心房颤动 (简称房颤 ) ,经服用胺碘酮后恢复窦性心律 ,心房部分感知不良。 1例术后心房电极即无感知功能。Holter随访中 ,1 3例心房感知率为 88.98%± 2 8.35 % ,1例术后心房感知正常的病人 ,随访时动态心电图示持续房颤。结论 :对于绝大多数窦房结功能正常 ,因房室阻滞安装起搏器的病人 。To evaluate the reliability of long term atrial sensing of VDD pacing s ystem,the single lead VDD pacing system was implanted in 14 patients with atriov entricular block(AVB) and normal sinus node function.Electrocardiogram was recor ded every day in the first week after implantation,24 hour Holter monitoring wa s taken in 14 patients during 32.50±13.20 months after implantation.During Holt er monitoring,patients sat,stood,took supine position,lay on right and left side ,respired deeply,coughed,took Valsalva maneuver and exercised for 2 minutes resp ectively,and the corresponding time was recorded.24 hour atrial sensing ratio w as checked manually.Results:Atrial sensing function was normal in 12 patients in the first week after implantation,and atrial fibrillation occurred in one patie nt in the third day after implantation,and after amoidarone treatment,sinus rhyt hm recovered,atrial sensing undersensing.Atrial sensing failed immediate after i mplantatio n in one patient.24 hour monitoring demonstrated the mean atrial sensing ratio was 88.98%±28.35% in 13 cases,permanent atrial fibrillation occurred in one p atient whose atrial sensing function was normal after implantation.No atrial ove rsensing was observed during 24 hour Holter monitoring.Conclusions:In most pati ents,the single lead VDD pacing system can maintain long term and stable atrial ventricular pacing.[ Chinese Journal of Cardiac Pacing and Electrophysiology , 2003,17(1):46～47]
