摘　　要：目的　研究普伐他汀对冠心病不稳定型心绞痛伴高脂血症患者血脂、内皮素及临床症状的影响。方法　76例冠心病伴高脂血症患者随机分为常规治疗组和普伐他汀治疗组 ,观察治疗 2个月、4个月时心绞痛的临床症状、血脂、内皮素的变化。结果　治疗 2个月时普伐他汀治疗组便出现总胆固醇 (totalcholesterol,TC)、甘油三酯(triglycerides ,TG)、低密度脂蛋白胆固醇 (low densitylipoproteincholesterol,LDL C)下降 ,高密度脂蛋白胆固醇 (high densitylipoproteincholesterol,HDL C)升高 ,内皮素 (endothelin ,ET)下降 ;常规治疗组 4个月时方出现上述变化。并且治疗 4个月后普伐他汀治疗组较常规治疗组血脂、内皮素改善更明显。普伐他汀组治疗 4个月后 ,发现ET分别与TC、TG、LDL C呈正相关。治疗 4个月后 ,普伐他汀治疗组心绞痛缓解率明显提高 ,且在调脂方面以胆固醇降低最明显。结论　普伐他汀降脂疗效显著 ,可减轻动脉粥样硬化的程度 ,改善血管内皮功能 。Objective To study the effect of pravastatin on blood lipid,plasma endothelin and clinical symptom of patients with unstable angina and hyperlipoproteinemia.Methods Seventy six patients with coronary disease and hyperlipoproteinemia were divided into trial group and control group randomly.Change of blood lipid,plasma endothelin and clinical symptom were observed after treatment of 2 months and 4 months.Results After treating 2 months total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL C) and endothelin(ET) decreased in trial group,while the decrease occurred in control group until 4 months.In contrast to control group,the blood lipid and ET in trial group improved obviously after 4 months,ET was positively related to TC,TG and LDL C respectively.Angina relieving rate of trial group was evidently higher than that of control group;on adjusting blood lipid,pravastatin decreased TC in the most effective way.Conclusion Pravastatin can decrease blood lipid dramatically,relieve atherosclerosis,improve endothelium function and enhance the total treatment efficiency rate for angina.
