病毒性心肌炎患者柯萨奇B组病毒特异性IgM抗体检测的应用

摘　　要：目的　早期诊断成人病毒性心肌炎柯萨奇B组病毒IgM抗体 (CBVIgM )。方法　采取免疫酶染色法 (酶染玻片法 )测患者血清CBVIgM抗体 ,并与病毒分离和微量细胞中和抗体测定相比较。结果　 12 9例心肌炎患者血清CBVIgM阳性 82例 ,阳性率 63 6% ;46例心肌炎患者咽拭子作病毒分离 ,分离出CBV 12株 ,其CBVIgM阳性有 11例 ;46例病毒分离患者的双份血清 2 6份与单份血清 (为急性期血清 ) 2 0份 ,其中和抗体≥ 4倍升高和单份血清≥ 1∶32 0者有 31例 ,31例中CBVIgM阳性为 2 9例。 40例健康对照组病毒分离和中和抗体均阴性 ,其CBVIgM阳性 2例。结论　免疫酶染色法检测CBVIgM ,方法简单 ,快速 ,可靠 ,其结果与对照组相比有非常显著差异 ,与病毒分离阳性符合率为 91 7% ,与血清中和抗体阳性符合率为 93 5 %。Objective To examine the specific Coxsackie B virus IgM(CBV IgM)for early is diagnosis of virus myocarditis.Methods Immunoenzyme dye process was used to examine the serum CBV IgM in patients and compare it with microcell neutralization antibody,and isolated virus.Results 82 of 129 patients with viral myocarditis were the serum CBV IgM positive and the positive rate was 63.3%,Using the throat swads as spcimens to separates virus,We separated 12 isolates of Coxsackie B virus in 46 patients and 11 patients were CBV IgM positive.In the 26 double serum samples and 20 sigle serum samples (the acute serum)of the 46 patients,neutralization antibody of 31 patients increased higer than 4 fold and sigle serum titer was higer than 1∶320,29 patients were CBV IgM positive.40 health people were as contral,whose neutralization antibody were negative and no Coxsackie B virus was isolated,but 2 people were CBV IgM positive.Conclusion The immunoenzyme dye process to examinate CBV IgM is simple,quick and reliable,the result compared with contral's has statistically significant difference and the rate of confermity with the viral separated positive rate is 91.7%,and that of confermity with the serum neutralization antibody positive rates 93.5%.
