摘  要:【目的】 探讨采用不透X线标志物检测胃肠传输时间对儿童功能性便秘的诊断及临床分型的意义。 【方法】  2 8名小儿功能性便秘患儿及对照组 68名正常健康儿童于检测第 1天上午 8时各口服不透X线标志物胶囊 1粒 ,便秘组于 2 4、48及 72小时摄腹部平片。对照组于 12、2 4及 48h摄腹部平片。根据骨性标志将腹部平片分为右半结肠 (RC)、左半结肠 (LC)及乙状结肠直肠 (RS)区域 ,并记录每个区域的标志物数目。 【结果】 功能性便秘组的全胃肠传输时间 (totalgastrointestinaltransittime ,TGITT)、口 盲传输时间 (mouth intestinetransittime ,M ITT)及节段性结肠传输时间包括右半结肠传输时间 (rightcolonictransittime ,RCTT)、左半结肠传输时间 (leftcolonictransittime ,LCTT)和直肠乙状结肠传输时间 (rectosigmoidcolonictransittime ,RSTT)较对照组明显延长 (80 .4± 2 3 .1)hvs(2 3 .6± 5 .6)h、(2 0 .7± 8.6)hvs (9.9± 2 .4)h、(2 0 .3± 7.5 )hvs (7.3± 3 .1)h、(12 .8± 3 .1)hvs (3 .4± 2 .8)h、(2 6.8±6.4)hvs (4 .1± 3 .2 )h ,P <0 .0 1)。 【结论】 根据不透X线标志物的分布与胃肠传输时间将功能性便秘分为 :慢传输型 ;功能性出口梗阻型 ;混合型。胃肠传输时间测定对判断全胃肠?In order to study the value of gastrointestinal transit test(GITT) in diagnosis of functional constipation in children with radiopaque markers(ROM). Radiopaque markers were given orally to 28 patients with functional constipation and 68 normal children at 8 am on days 1,Then a plain abdominal film taken at 24,48,72h and 12,24,48h respectively.Each film was divided into right colon(RC),left colon(LC) and rectosigmoid(RS) areas using bony landmarks,and the marker content of each area counted. The TGITT,M ITT,CTT of the normal children were (23.6±5.6)h,(9.9±2.4)h and (14.8±4.8)h respectively.The segmental colnic transit time include:the right colonic transit time(RCTT),the left colonic transit time(LCTT) and rectosigmoid colonic transit time(RSTT).The RCTT,LCTT and RSTT were (7.3±3.1)h,(3.4±2.8)h,(4.1±3.2)h respectively.In the constipated children,The TGITT,M ITT,RCTT,LCTT and RSTT were significantly longer than those in controls[(80.4±23.1)h vs (23.6±5.6)h,(20.7±8.6)h vs (9.9±2.4)h,(20.3±7.5)h vs (7.3±3.1)h,(12.8±3.1)h vs (3.4±2.8)h,(26.8±6.4)h vs (4.1±3.2)h, P <0.01]. [Conclusions] With the TGITT and CTT,constipation can be divided into three types:slow transit constipation,outlet constipation and mixed type.It could provide a reliable basis for the diagnosis of function constipation due to dysfunction of the total gastrointestinal or segmental colon.And this method is of practical value in confirmation of the disease cause of constipation and its treatment.
