摘 要:目的与方法 :通过观察轻 -中度发作期哮喘患儿麻疹疫苗反复注射前后IL - 1 2 ,IL - 1 3及血清总IgE水平的变化 ,探讨麻苗治疗对哮喘患儿的免疫影响及其治疗小儿哮喘的作用机制。采用生物素 -亲合素双抗夹心酶联免疫 (ABC -ELISA)法测定麻苗治疗前后 1 3例哮喘患儿 (麻苗治疗组 )血浆及外周血单个核细胞 (PBMC)培养上清中的IL - 1 2、IL - 1 3与血清总IgE水平 ,并与 1 2例单纯对症治疗组哮喘患儿及 1 7例正常对照组小儿比较。结果 :麻苗治疗后 ,IL - 1 2水平与对症治疗组无显著差异 (P >0 0 5) ,IL - 1 3与血清总IgE水平显著降低 (P <0 0 5)。血浆IL - 1 2水平与血清总IgE水平呈负相关 (r =- 0 437,P <0 0 5) ,PBMC培养上清中的IL - 1 2水平与血清总IgE水平无相关性 ;血浆及PBMC培养上清中的IL - 1 3水平与血清总IgE水平呈正相关 (r=0 657及r=0 485 ,P <0 0 1 ) ;血浆及PBMC培养上清中的IL - 1 2水平与IL - 1 3水平呈负相关 (r=- 0 460及r=- 0 383 ,P <0 0 5)。结论 :麻疹疫苗反复注射是通过下调哮喘患儿IL - 1 3水平 ,进而降低血清总IgE水平 ,而对IL - 1 2水平无显著影响。AIM and METHODS: To study the immunological effect of measles vaccine therapy on asthmatic children, we examined the changes of interleukin-12 , interleukin-13 and total serum IgE levels in plasma and cultured peripheral blood mononuclear cells(PBMC) supernatant by means of ELISA in 13 mild-moderate asthmatic children treated with measles vaccine. Results were compared with 12 anti-symptomatic treatment mild-moderate asthmatic children and 17 normal children control group. RESULTS:After measles vaccine treatment, IL-13 and total serum IgE levels decreased remarkably, statistically lower than that of group receiving only anti-symptomatic treatment. There was no statistical difference in IL-12 level between the two group. Correlation analysis: 1)IL-12 level of plasma was negatively correlated to the level of serum total IgE, there was no correlation of supernatant IL-12 in PBMC to the total serum IgE; 2)IL-13 levels in plasma and PBMC were positively correlated to the level of total serum IgE; 3) IL-12 level was negatively correlated to IL-13. CONCLUSION: Measles vaccine could down-regulate IL-13 level, therefore decrease total IgE synthesis, but not affect IL-12 level in asthmatic children.
