摘 要:目的 为了解母婴间血清前白蛋白(PA),白蛋白,IgG,总蛋白的关系。方法 用免疫单向检测法测定40对早产母婴及148对足月健康母婴血清PA,白蛋白,IgG及总蛋白的含量。结果 40例早产儿母亲血清PA,白蛋白,总蛋白显著高于早产儿脐血血清PA,白蛋白,总蛋白(P<0.001)。母亲血清IgG低于新生儿脐血血清IgG(P<0.05);148对足月儿母亲血清PA显著高于新生儿脐血血清IgG(P<0.01)。母亲血清IgG显著低于新生儿脐血血清IgG(P<0.01)。母婴间血清白蛋白及总蛋白无明显差异;各孕周母婴间血清IgG,总蛋白相关分析呈正相关。母婴间PA无相关性;早产儿组血清白蛋白,IgG,总蛋白,PA均非常显著低于足月儿组(P<0.001);新生儿体重与前白蛋白水平呈正相关。早产儿,足月儿体重与PA水平分别呈中度(r=0.41)和高度(r=0.58)相关。结论 孕妇的全面营养尤其是蛋白营养状况与其胎儿及新生儿的营养状况有非常密切的关系;PA的测定可以作为评估新生儿宫内或宫外蛋白营养状况的敏感生化指示。Objective: to study the relationship between maternal and neonatal serum prealbumin(PA),albumin(Alb),IgG and total proteins(TP). Methods:40 couples of mothers and their pre?term newborns and 148 couples of mothers and their full term newborns'serum PA,Alb,IgG and TP levels were determined by single direction diffusion immunoassary. Results: the PA, Alb and TP levels in pre?term newborns' mothers' serum are significantly higher than those in babies' umbilical serum separately (P<0.001),but mothers' IgG level is lower than the pre?term newborns' significantly (P<0.001);full term newborns' mothers' serum prealbumin level is significantly higher than their babies' significantly (P<0.01),but mothers' IgG level is lower than the full term newborns' significantly;there are positive relativities between mother' and newborns'(both pre?term and full term) serum Alb,IgG and total proteins levels separately, there is no relativity between mothers' and newborns' PA levels. All the index levels of pre?term newborns are lower than those of full term newborns separately (prealbumin P<0.001, others P<0.05); newborns' birth weight is positive relative to PA level(pre?term r=0.41,full term r=0.58). Conclusions: maternal nutritional status has close relationship to newborns', and serum prealbumin level can be used as a sensitive biochemical indicator on perinatal newborns' protein nutritional status.
