腘静脉外肌袢形成术在倒流性下肢深静脉机能不全及伴有腘静脉陷迫症时的临床应用

摘　　要：目的　探讨静脉外肌袢形成术在倒流性下肢深静脉机能不全及伴有静脉陷迫症时的临床疗效。方法　对 18例实施手术患者的临床资料进行回顾性分析。结果　全部患者临床症状均有不同程度改善 ,其中 15例疗效最为满意 ,占 83.3% (15 /18)。 5例合并静脉陷迫症的患者术后静脉造影均见原陷迫处血管通畅。术后无一例发生深静脉血栓形成 ,但其他近期并发症相对较多。结论　 (1)静脉外肌袢形成术是重度倒流或倒流Ⅲ～Ⅳ级的下肢深静机能不全的优选手术 ,其肌袢的长度是影响术后疗效的关键。 (2 )在诊断倒流病变的同时 ,应注意静脉陷迫症的存在 ,手术时应同时行陷迫松解 ,纠正回流障碍因素。 (3)严格掌握适应证及熟练、规范的技术操作是减少并发症的关键。Objective To evaluate the clinical effects of substitute valve operation for deep venous insufficiency and popliteal vein entrapment syndrome (PVES) of legs.Method The clinical data of 18 patients operated were analyzed retrospectively.Results All of the patients were improved in varying degrees after treatment.15 of the 18 patients(83.3%) achieved satisfying curative effect.The postoperative venography of 5 patients combined with PVES showed that the venous was unblocked.No one had deep venous thrombosis,but had more recent complications postoperativly.Conclusion (1)Substitute valve operation is the best pattern for the seriously refluent of deep venous insufficiency (in grade Ⅲ～Ⅳ) of legs,The length of the muscular loop is the factors influencing therapeutic effects.(2)The diagnosis of PVES should be seriously considered while refluency occured.The compression should be released during the operation.(3)Careful selection of the indications and proficient surgical skills are the keys for decreasing postoperation complications.
