选择性支气管动脉“夹心面包”栓塞治疗咯血的临床应用

摘　　要：目的评价不同的栓塞材料和操作程序对选择性支气管动脉“夹心面包”栓塞治疗咯血疗效的影响。方法5 8例咯血患者分组接受选择性支气管动脉“夹心面包”栓塞 (A组 )和单纯明胶海绵栓塞 (B组 )治疗。结果两组患者经过治疗后及随访 6个月至 3年 ,短期止血率和总有效率分别为 :A组 87.5 %、10 0 % ,B组 6 5 %、92 %。两组病例长期止血率相比较 ,差异有显著性 (P <0 .0 5 )。结论选择性支气管动脉“夹心面包”栓塞治疗咯血是一种止血迅速、疗效确切的治疗措施 ,能明显提高咯血的远期疗效。ObjectiveTo evaluate the effects of various embolic material and operative procedure on the therapeutic efficacy of selective 'sandwich' embolization of bronchial artery for treatment of hemoptysis.MethodsFifty(eight patients with hemoptysis were divided into two groups:①Group A received the selective 'sandwich' embolization of bronchial artery;②Group B received single gelfoam embolization.ResultsAfter following up for 1/2～3 years, the healing rate and total efficacy were 87.5% and 100% in Group A; 65% and 92% in Group B. By comparison, significant difference of cure rate existed between both groups (P< 0.05 ).ConclusionSelective 'sandwich' embolization of bronchial artery is a rapid and effective measure to control hemoptysis, showing excellent long term results.
