老年人2型糖尿病的危险因素分析

摘　　要：目的　分析与老年人 2型糖尿病 (DM)有关的危险因素。　方法　横断面调查法调查北京市西城区和海淀区的老年人 3 939例 ,用多因素logistic回归分析法对危险因素以及代谢异常的聚集数与DM的相关性进行分析。DM的诊断标准为空腹血糖≥ 7 0mmol/L或有糖尿病现患史。　结果　本调查共检出 487例糖尿病患者。统计结果显示DM家族史、年龄、血压、腹围和高密度脂蛋白胆固醇 (HDL C)与DM独立相关 ,OR(95 %CI)分别为 3 74(2 90～ 4 83)、1 0 3(1 0 1～ 1 0 5 )、1 16 (1 11～ 1 2 1)、1 0 2 (1 0 1～ 1 0 3)和 0 5 8(0 41～ 0 84)。随腹部肥胖、高血压、高甘油三酯(TG)、低HDL C这 4个代谢紊乱指标在个体内聚集数目的增加 ,DM的危险性亦逐渐增大。　结论　除遗传和年龄因素外 ,多个代谢紊乱指标的聚集与DM密切相关 ,控制代谢综合征对预防糖尿病具有重要意义。Objective To analyze the related risk factors for type 2 diabetes (DM) in the old people. Methods Cross sectional study was carried out in a sample of 3 939 ( 60～79 years old) in Xi Cheng, Hai Dian districts in Beijing. The diagnostic criteria for DM is fasting glucose ≥7.0 mmol/L,or the history of having ever diagnosed DM. Multifactor logistic regress analysis was used to determine the risk factors of DM and the association between the number of the clusters of metabolic disorders and the risk of DM. Results 487 cases of DM were found by this survey. The statistical results showed family history of diabetes, age, blood pressure, waistline and high density lipoprotein cholesterol (HDL-C) were independently associated with the risks of DM, with OR(95% CI) being 3.74(2.90～4.83), 1.03(1.01～1.05), 1.16(1.11～1.21), 1.02(1.01～1.03), and 0.58 (0.41～0.84),respectively. The risk of DM increased with clustering of abdominal obesity, hypertension, high triglyceride, and low HDL-C in individuals. Conclusion In addition to age and genetic factors, metabolic factors, especially their clusters are closely related to the risk of diabetes. Therefore, metabolic disorder intervention is suggested for DM control.
