急性坏死性胰腺炎时胃粘膜缺血与炎症介质相关性的实验研究

摘　　要：目的　探讨急性坏死性胰腺炎 (ANP)时胃粘膜缺血与炎症介质过度释放度的关系。方法　 SD大鼠 6 4只 ,随机分成对照组和 ANP组。 5 %牛磺胆酸钠胰腺被膜下均匀注射制成 ANP模型。采用放射生物微球技术在制模后 2小时及 12小时分别测定胃粘膜血流 (GMBF) ,同时检测血清磷脂酶 A2 (PL A2 )、白细胞介素 - 1β(1L- 1β)水平及胃粘膜病理变化。结果　ANP组大鼠在制模后 2小时及 12小时血清 PL A2 活性 (94 .2 9± 9.96 ,10 3.71± 14 .4 0 ) U/ L、IL- 1β水平 (0 .78± 0 .13,0 .83±0 .2 0 ) ng/ m l均较对照组 (6 5 .2 7± 10 .5 2 ,6 6 .6 3± 9.81) U/ L、(0 .32± 0 .0 6 ,0 .33± 0 .0 7) ng/ ml明显升高 (P值均 <0 .0 0 1)。ANP组 GMBF在制模后 2小时与 12小时均较对照组明显减少 ,GMBF与血清 PL A2 、IL- 1β水平之间呈密切负相关 (P<0 .0 5 )。结论　 ANP早期 GMBF的减少与炎症介质的增高同时发生 ,两者均是Objective To explore the relationship between gastric mucosal ischemia and inflammatroy mediators released in rats with acute necrotizing pancreatits(ANP).Methods A total of 64 rats were randomized into control group and ANP group.The ANP model was induced by injection of 5% sodium taurocholate under the pancreatic capsule.Radioactive biomicrosphere technique was used to measure the gastric mucosal blood flow(GMBF) at 2h and 12h after the induction of ANP,meanwhile,blood and gastric tissue sample were taken for observation of phospholipaseA 2(PLA 2),interleukin 1β(IL 1β)levels and gastric mucosal pathological changes.Results Comparing control group,the PLA 2 and IL 1β levels in serum increased significantly,the GMBF reduced significantly at 2h and 12h after induction of ANP(all P<0.001).GMBF was closely correlated with PLA 2 and IL 1β levels in serum(P<0.05).Conclusion The reduction of GMBF and the increasement of inflammatory mediators developed simultaneously in ANP early,hence both of them are important pathogenic factors of gastric mucosal injury in ANP.
