食管心房调搏负荷试验心率校正ST段变化对冠心病的诊断价值

摘　　要：目的探讨经食管心房调搏负荷试验(TEAPT)心率校正ST段变化对冠心病的诊断价值。方法观察55例正常对照组和38例冠心病组患者的试验结果,测定每一时点心电图ST段压低值,计算最大ST/HR斜率和△ST/HR指数,并比较其与传统ST段压低标准的诊断效能。结果正常组最大ST/HR斜率、△ST/HR指数和ST段标准的95%上限值分别为1.1μV/次/min、0.77μV/次//min和70μV,以此为界限值,最大ST/HR斜率诊断冠心病的敏感性、准确性显著大于传统ST段压低标准(P<0.05)。结论食管心房调搏负荷试验无创、安全、经济,最大ST/HR斜率诊断冠心病具有较高的准确性。Objective To study diagnostic criteria and power of adjusted ST segment depression by heart rate in transesophageal atrial pacing strees test(TEAPT). Methods Fifty five normal persons and 38 patients with coronary artery disease were received TEAPT. ST depression was recorded and measured at rest and each class end of atrial pacing. Maximum ST/HR slope and △ST/HR index was calculated respectively,diagnostic criteria and power of two parameters and ST depression was evaluated and compared. Results 95% superior limit of maximum ST/HR slope ,△ST/HR index and ST depression was 1.1 μV/bpm, 0.77 μV/bpm,70μV ,respectively in normal group. Using superior limit values as diagnostic criteria, sensitivity and accuracy of maximum ST/HR slope was significantly higher than that of ST depression(P<0.05). Conclusion TEAPT is a feasible, safety, cheap, maximum ST/HR slope was had a very nice diagnostic power in TEAPT for coronary artery disease.
