运用多水平模型分析哈密老年人生命质量影响因素

摘　　要：目的了解哈密地区老年人健康相关生命质量及影响因素。方法资料来源于哈密2013年卫生服务调查,采取多阶段分层、整群随机抽样方法。对健康相关生命质量影响因素进行单因素2检验和多因素Logistic回归分析。结果哈密老年人身体疼痛或不适存在问题比例最高,欧洲五维健康量表(eropean quality-5,EQ-5D)指数等于1的老年人743人,占69.0%,EQ-5D指数小于1的老年人334人,占31.0%,不存在高水平聚集性。EQ-VAS评分平均(68.64±14.70)分,良好389人,占36.1%,不良688人,占63.9%,存在街道(乡镇)水平聚集性。两周病伤、半年慢病、文化程度、年龄、婚姻状况、收入水平(P<0.05)是老年人健康相关生命质量的影响因素。结论应注重老年人慢性病预防与管理,宣传健康相关知识,加强对低收入老年人的政策扶持,提高老年人健康相关生命质量。Objective To investigate the health related quality of life status and its influencing factors among the elderly in Hami area. Methods Data were collected from Hami health services survey in 2013. Cluster sampling on various stages and strata method was adopted. Chi-square test was used in univariate analysis. Multivariate Logistic regression was used to analyze the risk factors for health related quality of life of the elderly. Results The pain / discomfort was the most common problem. There were 743 elderly people whose EQ-5D index was equal to 1,accounting for 69. 0%. There were 334 elderly people whose EQ-5D index was less than 1,accounting for 31. 0%. The EQ-5D index was not clustered in high level. The mean score of EQ-VAS was( 68. 64 ± 14. 70),389 people were well-being,accounting for 36. 1%,688 people were not in condition,accounting for 63. 9%. The EQ-VAS clustered in street( village) levels. The health related quality of life was associated with physical discomfort in the past two weeks,chronic disease,education,age,marital status and incomes levels( P < 0. 05). Conclusions Hami area should focus on the prevention and management of chronic disease,propagate the health-related knowledge. We should strengthen the policy support for low-income old people,so as to improve health-related quality of life of the elderly.
