摘  要:目的 探讨肝移植患者术后营养支持的方法。方法 根据 5例患者术前、术后的代谢特点 ,术前给予肠内营养 ,必要时辅以肠外营养支持 ;术后早期 (1- 4d)采用全肠外营养 (TPN) ,辅以白蛋白 ;肠道功能恢复后应实施肠内营养 (EN)与肠外营养 (PN)相结合的营养方案 ,7- 10d后恢复经口饮食。结果  5例患者顺利度过围手术期。结论 术前营养状态与肝移植后疗效密切相关 ,术前应适当、合理有效地给予营养支持。术后在胃肠功能恢复前给予全胃肠外营养 ,控制营养物质的供给量 ,以利于供肝功能的恢复 ;胃肠道功能恢复后 ,宜尽快开始EN ,同时结合PN ,尽早过度到经口饮食。Objective To study the methods of nutritional support for patients with after liver transplantation. Methods According the characteristic of the metabolism of the 5 cases, the enteral nutrition was used before operations, and the parenteral nutrition was applied when necessary. In the early postoperative period (1-4 d), the total parenteral nutrition (TPN) was applied. The enteral nutrition (EN) and parenteral nutrition (PN) were administrated immediately after the recovery of the intestinal function. Oral diet was given from 7 to 10 days after operation. Results The 5 cases survived the peri-operation period. Conclusions The effect of the liver transplantation is correlated with the pre-operative nutritional status. The appropriate and effective nutritional support before operation should be applied. Afer the operation, TPN can be used the gut. The nutritional supply should be limited to benefit the recovery of the graft. EN should be administrated as soon as the gut works and oral diet should be give as soom as possible.
