摘 要:目的 :探讨诱导痰液 (SI)检测方法在评价哮喘患儿气道炎症方面的意义。方法 :对 11例哮喘间歇期患儿的 SI、支气管肺泡灌洗液 (BAL F)中的炎症细胞、活化嗜酸性粒细胞 (EG+ 2 细胞 )、嗜酸性粒细胞阳离子蛋白 (ECP)、白介素 5 (IL- 5 )进行比较分析。结果 :SI中的有核细胞总数及中性粒细胞、嗜酸性粒细胞、EG+ 2 细胞百分数均较 BAL F中明显增高 [分别为 (16 .0 ± 2 .3)× 10 5/m l与 (8.1± 1.5 )× 10 5/m l,(2 9.2± 10 .3) %与 (4.5± 1.8) % ,(11.7± 4 .4 ) %与 (3.0± 1.2 ) % ,(8.2± 3.1) %与 (2 .0±0 .9) % ],SI中 ECP[(10 8.1± 116 .2 )μg/L ]明显高于 BAL F中 ECP[(7.2± 10 .5 )μg/L ],SI中 IL - 5 (5 9.2± 4 0 .8ng/L )明显高于 BAL F中 IL - 5 (<10 ng/L )。嗜酸性粒细胞、EG+ 2 细胞在 SI与 BAL F之间呈正相关 (均 P <0 .0 5 )。结论 :SI检测法可作为检测哮喘患儿气道炎症的一种可靠方法 ,SI中的细胞和可溶性介质比 BAL F的更密集 ,可检出气道分泌物中含量较低的化学物质 ,适用于临床动态观察 。Objective:To compare the variation of inflammatory cells and cytokines in both sputum induced(SI) and bronchoalveolar lavage fluid(BALF) in children with asthma in order to evaluate the usefulness of SI as a noninvasive way in monitoring of airway inflammation. Method:We collected the SI and BALF in 11 children with stable asthma for measuring and comparing the markers of infllammation. Result:The numbers of total cells,PMN,EOS,EG + 2 cell in the SI were higher than those in BALF[by (16.0±2.3)×10 5/ml to (8.1±1.5)×10 5/ml?(29.2±10.3)% to (4.5±1.8)%?(11.7±4.4)% to (3.0±1.2)%?(8.2±3.1)% vs (2.0±[FK(W]0.9)%] respectively). The percentages of EOS and EG + 2 cell in SI had a closer correlation with in BALF(P< respectively). The percentages of EOS and EG + 2 cell in SI had a closer correlation with in BALF(P<0.05). Sputum ECP(108.1±116.2 μg/L),IL 5 (59.2±40.8 ng/L)were significantly higher than BALF ECP(7.2±10.5 μg/L), IL 5(<10 ng/L).Conclusion:SI is a clinically practical and reliable way in studying and monitoring the changes of inflammatory cells and cytokines in asthmatics. It is not only noninvasive and easily repeated but also yields samples more concentrated and richer in airway secretions than those obtained by bronchoscopy
