脊髓型颈椎病合并胸椎管狭窄症的诊断与治疗

摘　　要：目的探讨脊髓型颈椎病合并胸椎管狭窄症的诊断与治疗。方法回顾性分析4例脊髓型颈椎病合并胸椎管狭窄症的诊断与治疗。其中2例先行颈椎减压和稳定手术,后行胸椎板切除减压术;1例颈、胸椎同时行减压术;1例先行胸椎板切除减压术,后行颈椎板成形术。结果神经病学损害完全恢复2例,明显改善2例。结论脊髓型颈椎病合并胸椎管狭窄症手术治疗可取得良好的效果。Objective To study the diagnosis and treatment of cervical spondylotic myelopathy accompanied by thoracic spinal canal stenosis.Methods The clinical data of4patients with cervical spondylotic myelopathy accompanied by thoracic spinal canal stenosis were retrospectively analysed.The decompression and stability of the cervical spine were first performed and the thoracic laminectomy then done in2cases.The decompression of the cervical and thoracic spinal cord was done at the same time in1case.The thoracic laminectomy was first performed and cervical open-door laminoplasty then done in1case.Result Of the4patients with cervical spondylotic myelopathy accompanied by thoracic spinal canal stenosis,2perfectly recovered from neurological impairment and2were obviously improved.Conclusion Cervical spondylotic myelopathy accompanied by thoracic spinal canal stenosis can be successfully treated by the operation.[
