选择性顺行脑灌注技术在主动脉全弓替换术中的应用(附3例报告)

摘　　要：目的 :总结选择性顺行脑灌注技术在主动脉全弓替换术中的临床应用体会。方法 :3例行主动脉全弓替换术的患者应用经无名动脉和左颈总动脉插管的方式进行选择性顺行脑灌注 ,脑灌注流量为 2 0ml/(kg·min)。结果 :选择性顺行脑灌注时间 63～ 92min(平均 80min) ,停循环时间 3 0～ 65min(平均 4 6min) ,术后恢复顺利 ,6～ 8h清醒 ,1 2～ 4 8h拔除气管插管 ,未见脑及脊髓并发症。结论 :在主动脉全弓替换术中应用选择性顺行脑灌注技术可以更加安全地延长深低温停循环时间 。Objective:To discuss clinical experience with selective antegrade cerebral perfusion in aortic arch replacement operation.Methods:Three cases with aortic arch replacement operation used selective antegrade cerebral perfusion via innominate artery and left common carotid artery. Cerebral perfusion volume was 20ml/kg/min.Results:Selective antegrade cerebral perfusion time was 63-95 minutes(mean 80). Circulatory arrest time was 30-65 minutes(Mean 46).The patients woke up 6-8 hours after operation,tracheal tube was detubated 12-48 hours(mean 20) after operation. Cerebral and spinal complications were not found.Conclusion:Selective antegrade cerebral perfusion in aortic arch replacement operation is more safe to prolong deep hypothermic circulatory arrest time and more effective to protect brain.
